Page 1 of 3 INSTRUCTOR - VISIT ONLY REQUIREMENTS Initial and Annual Once student placement is confirmed with the Learning Resources (LR) Student Placement Coordinator, follow the directions below. Click on the GMC WEBSITE link www.gwinnettmedicalcenter.org, then Programs and Classes, Student Experiences, “Step : Orientation Requirements” 1. Read “Part I – Faculty Orientation Requirements” & “Part 2 – Processing Student Orientation Materials”. 2. Email lhorst@gwinnettmedicalcenter.org your last 4 Social Security numbers. This will allow us to request access for you to do your required orientation online in our Learning Management System called MyNetLearning (NL). Allow a few days for system processing and you will receive an email with login/password and further directions for NL. Once you have your login/password to NL, go back to “Part I – Faculty Orientation Requirements” Under this section, in the second paragraph, click on the link to login to MyNetLearning. a) Further directions will accompany the email with your login/password information. b) Review CBLs “Instructor (VISIT ONLY) Initial and Annual CBLs” in NL. c) Do the test/acknowledgement – results are recorded into your NL transcripts. d) You are required to do these CBLs initially and annually to visit on site. 3. Turn in to the LR Student Placement Coordinator – 1. Copy of your licensing credentials. 2. ID Badge Form (See last page). Read the following closely: BADGES & Process to Obtain ID from the school REQUIRED at all times. GMC INSTRUCTOR ID/PARKING ACCESS Badge REQUIRED at all times. 1. Complete the ID/Badge form attached to this document. You will need a current picture of yourself saved as a “jpeg”. If you do not have such a picture, you will need to arrange to have your picture taken by our Security team.* 2. Send the above form and picture to Linda Horst, lhorst@gwinnettmedicalcenter.org who will send all to Security to be processed. (If you do not have a picture saved as a “jpeg” Linda will let Security know you will arrange a time to have your picture taken with them*.) Page 2 of 3 3. You will be responsible to pick up your badge at Security. Your school may also assign this responsibility to you for your students’ badges. 4. Your badge will give you access to parking and the units. There will be no charge for Instructor badges, however if they are lost there is a $25 replacement fee. Security asks that you park in Employee Parking. 5. Once you have your badge, your school informs L. Horst if you may send your students Parking Form and their picture to Security which will allow you to pick up the student badges from Security.*. (Students pay $10 for the badge and when returned at the end of the experience $5 is returned.) *ID Badges can be obtained from the Security office Monday- Thursday, 7:30am-4:00pm, Friday, 7:30-1:00pm. Miscellaneous DRESS CODE: In addition to wearing your school ID and the Gwinnett Medical Center “INSTRUCTOR” ID Badge, dress business casual or wear your school uniform. When is the hospital wear closed toed shoes and hose. See next badge for important GMC Picture ID Badge information Page 3 of 3 INSTRUCTOR/STUDENT Picture ID CARD ACCESS CONTROL Student Directions: Complete this form and give it to your instructor along with a $10 deposit and an electronic picture* of yourself to submit for a GMC Student Picture ID badge. ($5 is refunded upon return of your badge.) If your instructor is not coming on campus follow #1below for instructor badges. Instructor: 1. For your Instructor badge: submit this completed form and an electronic picture* of yourself to the Student Placement Coordinator in Learning Resources (LR), lhorst@gwinnettmedicalcenter.org . These documents will then be forwarded by LR to the Public Safety Office and you can arrange a time to pick up your badge at the Safety Office. Instructor Badges are free. 2. For your Student Badges: Once you have been authorized by LR to pick up student badges, please submit the student completed forms and electronic pictures* (JPEG format is preferable) to the Public Safety Office at Gwinnett Medical Center mshafer@gwinnettmedicalcenter.org; and let them know when you would like to pick up the badge/s. Be sure to bring a $10 cash deposit for each student badge. (When you return the student badges to the Public Safety Office they will refund you $5 for each student badge returned.) *If an instructor or student needs a picture made, call 678-312-4057 to arrange a time for the picture to be taken.
