School Referral Procedures (6-12)
When a student’s teacher or instructional team
feels that more support or intervention is
necessary, the school should involve the student
in the school’s RTI process or initiate student
support meetings using the Team Request for
Support and Problem-solving Intervention
Process packet (pages 1-6). This packet is
completed by the referring student team, and
NOT a guidance counselor.

This completed Team Request for Support
Request and Problem-solving Packet is sent to
the grade-level counselor. The counselor will
review the packet and put together an RTI
problem-solving/intervention team and set up
Meeting 1. This Intervention team must meet
AT LEAST 3 times. Meeting forms are
provided to guide the team through the RTI
process.
Meeting 1 - At the first meeting, the packet is
reviewed and the RTI/Student Intervention Team
Conference Form must be completed, including
interventions developed for implementation in
the classroom and school. Documentation of the
interventions is required and should be recorded
on the RTI/Student Intervention Record or some
other defined data recording record. Data must
be collected to show the effectiveness or
ineffectiveness of each intervention
implemented.
Parent Request Procedures
When a parent requests an evaluation, that
request must be communicated to the student’s
guidance counselor.
 The request can be verbal or in writing
(any format) and can be directed to a
teacher, therapist, administrator or other
school personnel.
The student’s RTI/SIT Chairperson/designee
meets with the parent to:
 Complete question 1 on page 1 of the
Comprehensive Student Review Packet
Cover
 Gather copies of any documents provided
by the parents (i.e., medical reports,
evaluations, prescriptions)
 Complete social history with the parents
 Provide the parents with a copy of ESE
Parental Rights Booklet
The parent should be given an explanation of
the school’s intervention/RTI process, and
asked if they would choose to have their child
participate in the RTI process rather than go
ahead with the evaluation at this time.
If the student has not been receiving
interventions, the data collected will be
present level of functioning. Interventions
should be initiated during the referral process
and data included in the ERCC Meeting.
Meeting 2 – The team will review and record

results of interventions on the RTI/Student
Intervention Team Follow-up Meeting Form, as
well as recording the next action the team plans
to take. Actions available to the team are
defined on the Follow-up Meeting Form. Please
note that the team can begin to request
screenings at this second meeting. Those
screening include hearing, vision, speech, and
medical/social history. Other screenings may
also be requested, based on individual student
need. The results of these screenings should be
discusses and considered at subsequent
meetings.
Meeting 3 and subsequent meetings – The
team will continue to use the RTI/Student
Intervention Team Follow-up Meeting Form, to
review and record intervention results, request
additional information and screenings, and to
record team decisions. Meeting 3 IS NOT the
ERCC Meeting.
Sustained and continued lack of progress or
achievement and other data collected during the
RTI/ School-based Support and Intervention
Process may indicate a specific area of
disability.
If the team wants to consider a referral for
evaluation, the student’s RTI/SIT Chairperson

completed in a timely manner – generally, no
more than 2 weeks.

compliance check and data review for evidence
of suspected area of disability prior to the ERCC
Meeting. If speech is the leading concern, the
Review Packet is sent to the Speech Language
Pathologist (SLP).

If items are missing, incomplete, insufficient or
interventions will continue, the RTI Team can
resubmit after corrective actions or interventions
are completed.

scheduling of the ERCC meeting is the
responsibility of the student’s RTI/SIT
Chairperson.
