Pharmacist - Infectious Disease Specialty (ID) (Full-time, Days) Roseville, CA The Infectious Diseases Pharmacist delivers pharmaceutical care through the provision of patientcentered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost efficient medication therapy in the specialty area of infectious diseases. Principal Accountabilities Performs Review of all therapeutic plans regarding antimicrobial agents Communicates and makes recommendations to staff on infectious disease medications and usage Responsible for data and support on antibiotic stewardship Communicates often (maybe even daily) with Infectious Disease Physician(s) Compounds and dispenses medications accurately for delivery to patient care areas. Documents intervention, outcomes and performance improvement. Manages medication therapy/protocols as a part of physician-pharmacist-nurse team. Provides functional supervision to supportive personnel. Provides medication information and education to medical staff and patients. Qualifications: Education: Graduate from a college of Pharmacy accredited by the American Council on Pharmaceutical Education. PharmD and Post Graduate Residency (PGY-1) are desirable. Secondary residency (PGY-2) in Infectious Disease and/or antibiotic stewardship highly desirable Licensed as a Pharmacist(R.Ph) in the state of California. Experience: Previous experience in a hospital/acute care, trauma setting is desired. Experience or work within the Infectious Disease specialty highly desired and should be stated clearly on resume / application. Knowledge: Requires an in depth knowledge and the ability to apply legal requirements and accreditation standards including TJC (The Joint Commission), Title XXII, DHS, DEA, FDA and USP. Skills: Requires critical thinking skills, decisive judgment, delegation skills and the ability to work with minimal supervision. Must be able to work in a complex and dynamic environment and take appropriate actions in emergent situations. Must be able to communicate in an effective, tactful and diplomatic manner. Must have teamwork and delegation skills. Must have the skills to produce accurate work results within established timeframes.
