TITLE OF POSITION:
SCHOOL COMMUNITY OUTREACH COORDINATOR
Reports to:
Directly: Director of Student Supports
Indirectly: Principal
Basic Function:
In a Catholic environment that fosters collegiality and supports continuous learning, this employee is responsible for
the overall, non-academic success and development of Mother Teresa Middle School (MTMS) students. The School
Community Outreach Coordinator (SCOC) will work closely with administration, the Graduate Support Director,
MTMS staff, parents/guardians, outside agencies and the community to ensure the proactive, emotional, social and
physical development of students. The SCOC will act as an advocate, councilor and mentor as well as participate in
school activities and demonstrate a commitment to all aspects of the MTMS mission. The goal of the position is to
assist the Mother Teresa Middle School staff in developing students who demonstrate affective success, physical
health, character, leadership and service to others.
MTMS is a strength based school and the School Community Outreach Coordinator will use a strengths based
model that uses the strengths and resources of the client in the service of helping them achieve their goals and
realize their dreams.
The School Community Outreach Coordinator will design an internal and external, proactive school and community
service program targeted at affective success of MTMS students.
Expected Competencies:
 Work collegially with staff and administration to provide the school with proactive, strength based student
support.
 Work as part of the MTMS team to ensure student success in non-academic areas of development.
 Apply and follow up on crisis intervention protocol regarding child abuse, tragic events and inter-agency
involvement.
 Lead in the planning, organization and delivery of after school enrichment opportunities for students based
on interests and strengths.
 Lead in the planning, organization and delivery of summer programming based on student needs and
interests.
 Deliver first level, school based counseling to students and families.
 Provide access to external support programs and counseling services.
 Support the learning program by identifying the social and emotional needs of students/families and offer
effective solutions.
 Prepare individual, non-academic, affective development plans for each student and document case study
data on at risk students.
 Prepare and monitor data on student growth in the affective domain.
 Coordinate and process the hope, engagement and wellbeing student survey.
 Liaise with families to ensure active parent engagement and fulfillment of School/Parent Covenant.
 Assist Graduate Support Director with monthly parent evenings
 Serve as a mentor and onsite individual and group counseling.
 Assist staff in offering care for the whole person (cura-personalis).
 Work with staff to ensure students health needs are being addressed and aid in securing the appropriate
services to address these needs (dental, vision hearing).
 Act as a first level support for student hygiene concerns.
 Oversee the scheduling of student appointments.

Facilitate community service/social justice opportunities for students.
Coordinate the utilization of volunteers within the school.
Facilitate cultural opportunities for students within the community.
Co-ordinate the delivery of the school based nutrition program.
Oversee the transportation of students to and from school.
Assist in the planning and delivery of extra-curricular activities.
Coordinate and plan the monthly parent night.
Coordinate and plan volunteer appreciation events.
Oversee the procurement and distribution of school clothing.
Assist the GSD in the monitoring of student attendance.
Assist the GSD in identifying perspective students in the community.
Assist the GSD in the delivery of a school based mentorship program.
Assist in the organization and development of a School Parent Council.
Work collegially with potential nursing or social work practicum students.
Qualifications:
 Bachelors of Social Work Degree (BSW) or equivalent
 Successful experience working in an inner city school setting
 Experience working with at risk youth
 Formal training in policy practice, social research, vulnerable life conditions, death, grief, mediation and
restorative justice training
 Strengths assessment training in student affective intervention
 Certified parent project facilitation
 Excellent organizational skills and proven initiative
 Must be able to work within a budget
 Success in working with middle years, aboriginal and immigrant youth
 Success with working with an at risk population
 Excellent communication and presentation skills
 Advanced computer literacy and the proven ability to analyze student data
 Successful experience as a youth mentor
 Have access to a registered vehicle and a valid driver’s license

 Confidentiality
o At no time should the Social Worker discuss publicly or release information pertaining to
unauthorized school affairs. Any breach of confidentiality in this regard is a serious violation of
acceptable professional conduct.

Independence
o The School Community Outreach Worker is expected to work independently.

Client/Peer Contact
o This employee works collegially with other staff, teachers, school-based administration and schooldivision staff. Direct contact with students, parents and the general public is regular and frequent.
