SUMMARY OF CHANGES FOR 1 NOVEMBER 2015 AMA LIST OF MEDICAL SERVICES AND FEES This document sets out the changes made to the AMA List in the 12 months to 1 November 2015 and the date the changes took effect. These changes have been included in the printed version of the 1 November 2015 AMA List. 15 June 2015 Therapeutic Procedures Assisted Reproductive Services – items BH620 for oocyte retrieval, BH625 for transfer of embryos or both ova and sperm, and BH635 for preparation of semen have been amended by replacing the word “conjunction” with “connection”. Haematology – items BJ710 and BJ720 for autologous blood injection services have been amended by replacing the word “administration” with “transfusion” Botulinum Toxin: o 18 items (BM800 – BM845) have been collapsed into 13 items by specifying the full name of the applicable medicine i.e. Botulinum Toxin Type A Purified Neurotoxin Complex (Botox), Clostridium Botulinum Toxin Type A Toxin-Haemagglutin Complex (Dysport) or IncobotulinumtoxinA (Xeomin) and removing the age requirement; o As a result, three items BM801 for treatment of hemifacial spasm, BM825 for blepharospasm and BM835 for bilateral blephasrospasm in patients 18 years of age or older have been deleted; and o items BM806 for equinovarus foot deformity and BM808 for equinovalgus have been consolidated into item BM804 for enquinus; and o new item BM842 for intravesical injection of Botox for the treatment of urinary incontinence due to idiopathic overactive bladder. Surgical Operations Gynaecological: o item HA140 for vulvoplasty or labioplasty has been amended to cover the surgical repair of female genital mutilation and major congenital anomalies of the uro-gynaecological tract; and -1- o new item HA142 for localised gigantism which is causing significant functional impairment. Urological – items HC370 for cystoscopy and HE670 for periurethral or transurethral injection for the treatment of urinary incontinence have been amended to ensure that a service to which item BM842 applies cannot be claimed with these items. Plastic and Reconstructive – five items MJ475, MJ485, MJ505, MJ515 and MJ555 for rhinoplasty have been amended to include reference to clinical indications to clarify not for cosmetic purposes. Diagnostic Imaging Computed Tomography - item OD038 for cone beam CT of teeth and supporting bone structure has been amended to specify for dental and temporo-mandibular joint imaging. Pathology Chemical: o items PB090 and PB095 for HbA1c testing has been amended by replacing the word “glycosylated” with “glycated”; o new item PB097 for diagnosis of diabetes mellitus; o items PB170 and PB175 for serum B12 and/or red cell folate testing have been replaced by separating out vitamin B12 and folate testing into three new items PB655 and PB656 for vitamin B12 and item PB657 for folate testing; and o items PB185 and PB186 for vitamin D testing have been replaced with four new items PB650 and PB651 for 25-hydroxyvitamin D, and PB652 and PB653 for 1,25-hydroxyvitamin D testing. Genetics: o item PG055 for Kirsten ras (KRAS) gene mutation testing in patients with metastatic colorectal cancer has been amended to include testing for rat sarcoma oncogene (RAS) gene mutation and to clarify stage of colorectal cancer, including a fee increase; and o two new items for testing for RET gene mutation; item PG065 for patients with a suspected clinical diagnosis of multiple endocrine neoplasia type 2 (MEN2) and item PG066 for asymptomatic relative of a patient with a documented pathogenic germline RET mutation. -2- 1 November 2015 Professional Attendances Consultant Physician in Geriatric or Rehabilitation Medicine: o Case conference – item AJ200 to coordinate a case conference has been amended to include attendance by a specialist and restriction to the number of times the attendance can be billed to any particular patient over a 7 day period. Diagnostic Procedures Cardiovascular: o items AV210, AV220 and AV235 for implanted pacemaker and defibrillator testing involving electrocardiography have been amended to reflect new items AV213, AV216, AV233 and AV234 for remote monitoring of implanted cardiac device; and o four new items AV213 for implanted pacemaker remote monitoring, AV216 for implanted pacemaker testing, AV233 for implanted defibrillator remote monitoring, and AV234 for implanted defibrillator testing. Gastroenterology and Colorectal – new item BB006 for catheter-free ambulatory pH monitoring for gastro-oesophageal reflux disease. Radiation Oncology Brachytherapy Planning and Verification – new item BR875 for targeted intraoperative radiotherapy using an intrabeam device for treatment of early-stage breast cancer. Anaesthesia Therapeutic and Diagnostic Services – item CV230 for pundendal nerve has been amended by including the words “and or dorsal nerve”. Surgical Operations Paediatric Surgery Services 37 items have been amended to specify the eligibility age by including the words “on a person 10 years of age or older”. As a result, new items have been included to mirror the existing items, with a 30% fee increase, for when performed on a child under 10 years of age. General Surgery: o General: item EA345 and new item EA346 for excision of pre-auricular sinus; -3- o o o o item EA985 and new item EA986 for removal of branchial cyst; and item EA995 and new item EM996 for removal of branchial fistula. Endocrine surgery - item EC570 and new item EC571 for removal of thyroglossal cyst and/or fistula. Upper gastrointestinal surgery: item EJ015 and new item EJ016 for caecostomy, enterostomy, colostomy, enterotomy, colotomy, cholecystostomy, gastrostomy or gastrotomy; item EJ025 and new item EJ026 for laparotomy involving division of peritoneal adhesions where no other intra-abdomnal procedure is performed; item EJ035 and new item EJ036 for laparotomy involving division of adhesions in conjunction with another intra-abdominal procedure; item EJ105 and new item EJ106 for diagnostic laparoscopy; amended item EJ686 and included new item EJ687 for nonendoscopic insertion of gastrostomy button; item EK215 and new item EK216 for closure of enterostomy or colostomy; item EK225 and new item EK226 for refashioning of colostomy or ileostomy; item EK255 and new item EK256 for resection of small intestine with anastomosis; item EK285 and new item EK286 for appendicectomy; item EK295 and new item EK296 for laparoscopic appendicectomy; and item EK485 and new item EK486 for repair of congenital diaphragmatic hernia. Hernia and hydrocele: item EM210 and new item EM211 for repair of femoral or inguinal hernia or infantile hydrocele; and item EM220 and new item EM221 for repair of strangulated, incarcerated or obstructed hernia. Miscellaneous: item EM720 and new item EM721 for exploration of spermatic cord; item EM760 and new item EM761 for arrest of haemorrhage following circumcision; item EN155 and new item EN156 for removal of benign tumour of soft tissue by surgical excision; item EO035 and new item EO036 for removal of soft tissue and lymph nodes from one side of the neck; and item EO250 and new item EO251 for laparoscopic splenectomy. -4- Vascular: o Operations for vascular access: item EW690 and new item EW691 for central vein catheterisation by open technique; item EW700 and new item EW701 for central vein catheterisation by percutaneous technique; and item EW710 and new item EW711 for removal of central venous line or other chemotherapy device by open technique. Urological: o Paediatric genitourinary surgery: item HF400 and new item HF401 for excision of patent urachus; item HF410 and new item HF411 for orchidopexy for undescended testis; item HF420 and new item HF421 for orchidopexy for undescended testis in inguinal canal. item HF430 and new item HF431 for revision of orchidopexy for undescended testis; item HF440 and new item HF441 for exploration of groin for impalpable testis; item HF450 and new item HF451 for examination under anaesthesia for hypospadias with erection test; item HF460 and new item HF461 for glanuloplasty hypospadias incorporating meatal advancement; item HF470 and new item HF471 for first stage distal repair of hypospadias; item HF480 and new item HF481 for first stage proximal repair of hypospadias; item HF490 and new item HF491 for first stage repair of hypospadias; and item HF510 and new item HF511 for repair of post-operative urethral fistula for hypospadias. Ophthalmology - item MC020 and new item MC021 for excision of periorbital dermoid cyst. Other Surgical Changes General Surgery: o Miscellaneous - item EM770 for reduction of paraphimosis has been amended to include the words “or phimosis”. o Breast surgery - new item EO516 for complete local excision of malignant tumour of the breast with targeted intraoperative radiotherapy. Urological: o Operations on bladder (open) - item HC815 for formation of mitrofanoff continent valve has been amended by replacing the words “mitrofanoff continent valve” with “continent catheterisation bladder stomas (e.g. Mitrofanoff)”. -5- Paediatric: o Neonatal surgery: items MD310, ME400, ME410, ME420 and ME430 for hirschsprung’s disease have been amended by replacing the words “hirschsprung’s disease” with “aganglionosis coli”; and item MD470 for secondary operation for gastroschisis has been amended by including the words “or exomphalos”. o Abdominal surgery: 2 items ME440 for examination of the rectum and ME450 for submucosal or perirectal injection for rectal prolapse have been amended to restrict the eligibility age to over 2 years of age; As a result, 2 new items ME441 and ME451 have been mirrored for where the service is performed on a patient under 2 years of age; and 3 items ME460, ME470 and ME480 for inguinal hernia repair have been amended to extend the age eligibility from less than 3 months to less than 12 months. Diagnostic Imaging Computed Tomography - items OD376 and OD377 for computed tomography colonography (CTC) have been consolidated into new item OD378 for CTC for exclusion or diagnosis of colorectal neoplasia. Pathology Tissue Pathology – 2 new items PE085 for second expert opinion and detailed written report on a patient sample for no more than 30 mins and item PE086 for greater than 30 mins; and Genetics - item PG067 for fluorescence in situ hybridisation (FISH) testing in patients with non-small cell lung cancer to identify anaplastic lymphoma kinase (ALK) gene rearrangement status. 14 October 2015 -6-
