GLOUCESTERSHIRE YOUNG CARERS JOB DESCRIPTION JOB TITLE: Specialist Family Worker (Mental Health) LOCATION: 7 Twigworth Court Business Centre GRADE: Grade H (Pts 23-31, £20,198-£26,276) pro rata HOURS: 30 – 37 hours per week - 12 month contract 1. JOB PURPOSE To deliver whole family support to families where a parent experiences mental ill health, in order to reduce or prevent excessive and harmful care giving by young carers. To work with GYC development lead and key personnel in the 2gether NHS Foundation Trust to promote a whole family approach and to capture learning in order to inform service delivery in both organisations and beyond. 2. MAIN DUTIES AND RESPONSIBILITIES To reduce excessive and harmful caring roles of young carers by: a. attending a range of 2gether NHS Foundation Trust team meetings in order to promote the use of whole family tools and increase identification of service users who are parents and whose children may be taking on caring roles. b. establishing a trusting relationship with parents, young carers and their families referred by 2gether NHS Foundation Trust. c. Assessing need and delivering a package of whole family support within an agreed intervention framework including, as appropriate: a) promoting and supporting open communication within families in relation to the impact of mental health issues b) supporting families to complete the young carer crisis plan ‘Safe, Sorted and Supported’. c) supporting improvement in family connectedness d) supporting young carers to access information e) supporting parents to develop strategies which support parenting and minimise the impact of their mental health on dependent children f) supporting family / school communication g) evaluation d. engaging with the CAF process to ensure a multi-agency approach in supporting the whole family. e. Supporting young carers and parents to engage with GYC’s ‘Strength in Families’ project including advocacy for family members in Family Group Conferences (training will be provided) To contribute to the learning of the sector by: a) Routinely reviewing, monitoring and evaluating young carer and family outcomes using an accredited outcomes tool b) Gathering and collating evidence to inform Social Return on Investment c) Supporting skills development of GYC and 2gether Trust practitioners Other Duties a) undertake relevant and appropriate administrative work b) produce written reports as required e.g. Carers Trust, case conferences c) attend relevant meetings both internally and externally. d) represent GYC in a manner consistent with the highest professional standards reflecting GYC’s commitment to diversity and equality of opportunity. e) maintain continuing professional development to meet the changing demands of the job. f) keep abreast of, and follow at all times, national and local policy in relation to safeguarding children and vulnerable adults. g) undertake other duties which would reasonably fall within the remit of this post. 3. ACCOUNTABILITY Responsible to line manager within a supervisory setting Responsible for providing reports as required Responsible for providing monitoring information as required by the post’s funding source(s). 4. SPECIAL CONDITIONS Car driver with use of car, or with full access to equivalent transport. Flexible working hours, availability outside school hours is essential.
