I : Innovative Research Programmer Development
Basic
1993-1996 : In collaboration with Dr.Earl Silverman of the Division of Rheumatology, and with
Main operating grant support, I developed a programme for the immunologic, echocardiographic, electrocardiographic and outcomes assessment of the fetus or neonate affected by maternal aut immune disease.
1995-2000 : I developed a reproductible animal model (Langendroff rabbit) of congenital heart block to provide for detailed assessment of the antibody and antigen responsible for this disease.
1997-2000 : I have developed of an in vivo autoantigen-immunized mouse model of congenital complete heart block, and adapted an Embryonic Cardiac Doppler technique for assessment of rhythm in this model. A March, 1997 MDC operating grant application was highly rated (3.9-4.0).
The requested pilot data was achieved through a Pediatric Consultants grant and a reapplication was submitted (March 1, 1998) and resulted in a successfully funded 3-Yr MRC award which began in
November of 1998.
1997-2000
II : Innovative Research Development Programme Development
Applied
1999-2000 : I have investigated technologies for non-invasive fetal ECG acquisition using customized hardware and software. I am evaluating and adapting a device designed by MEDCO to determine it's utility in fetal heart disease and rhythm disorders. This project is the basis for all funded patient-based research seed grant (2 years, beginning April, 1999), and pilot data obtained has formed the basis for a multi-centered Heart and Stroke Foundation operating grant application.
This application recently missed external funding by one slot, and will be appropriately revised to obtain funding in the September, 2000 competition.
1992-2000 : I initiated the application of signal-averaged electrocardiography to children, including the development and publication of clinical normal values for 5 to 15 year olds. This technique saw multiple cardiac electrical depolarization signals (QRS), allowing for the assessment of late sign indicative of cardiac conduction delays. These "late potentials" are markers for areas of disease myocardium, such as in cardiac transplant rejection, right ventricular dysplasia, and other cardiac conditions predisposed to arrhythmias. These applications have been summarized in the
Australian MD thesis of Dr.Andrew Davis. The sensitivity and specificity of late potentials in the diagnosis of pediatric arrhythmogenic right ventricular dysplasia is now being assessed.
1999-2000 : Just as the ventricular QRS signal can be averaged for the detection of late potential atrial P-wave signal-averaged ECGs can be assessed. We have therefore begun to assess whether
P-wave signal-averaged measurements can differentiate patients with or without atrial for episodes following complex atrial repairs of congenital heart disease.
1999-2000 : The assessment of measures of heart rate variability (HRV) have become important for understanding the relative autonomic influences on the heart, and in predicting outcomes such as sudden cardiac death. We are evaluating the application of clinical HRV software to the assessment of HRV measurements in children. In particular, we are investigating two particular groups where automatic abnormalities are identified or suspected:the pediatric patient post-transplant, and the
pediatric patient undergoing tilt table testing.

III : Innovative Research Programme Development
Clinical
1999-2000 : In collaboration with Dr.Allan Coates, we have combined the cardiac and pulmonary exercise laboratories into a single cardioplumonary exercise unit and are expanding the capabilities and scope of the lab to support bot clinical care and clinical investigation. We have developed the ability to assess pulmonary variables, anaerobic threshold and peak VO2 and step state cardiac output, and are developing the ability to measure beat to beat indices of cardiac outputs and diastolic function using impedance cardiography.
I have developed three protocols for the assessment of all these factors in subgroups pf fontan patients (one co-developed with Dr.V.Arsdell), and have been funded to complete these studies.

3D Mapping: Enhancing safety in Pediatric Catheter Ablation
The surgical repair of congenital heart disease frequently results in the late complication of complex forms of atrial flutter. These complex atrial arrhythmias usually differ from "typical" atrial flutter seen in adults, but are frequently treatable by catheter ablation techniques. Because of their complexity, it is desirable to have a three-dimensional technique to display the anatomical substrate of the atrial endocardium and its sequence of activation.
The ideal system is inexpensive to use, easily applicable to a wide varity of cases, and allows for the use of any ablation technology (hot-tip or cold-tip). I run the only Canadian center dedicated to 3D mapping and ablation in pediatric arrhythmias, and have evaluated, acquired and developed a successful experience using multiple mapping systems including those based on magnetic positioning (CARTO), inverse solution mapping (ESI) and impedance-based catheter positioning
(Localisa and NAVX). By working with the latter technology, I have been able to reduce the number of lesions delivered (increasing safety) and accurately deliver insurance lesions (incresing efficacy).
Our use of this technology allows for the use of our crocatheter technology in ablation of these complex pediatric arrhythmias, which is a novel application. At the same time, we are comparing the anatomical accuracy of each of these techniques.

Cardiac Genetic Clinic, DNA Bank and Gene Sequencing Program
I developed a Cardiac Genetics Clinic and DNA banking program for both research and clinical care in 1998 with collaborators Peter Ray and Steve Scherer . Families with high-risk heritable heart conditions are seen and assessed as a unit. Proband DNA is banked and up to 2004 was sent to research colleagues either within or outside HSC depending on the suspected condition. For large family cohorts with undiagnosed familial conditions, candidate genes have been sequenced locally through collaboraton with Dr. Steve Scherer (PRKAG2, HCN4, PKP2, DSG2, DSP). Beginning in
2004 with the advent of North American clinical testing sites for many heritable arrhythmia conditions, we initiated a streamlined urgent process for assessing high risk conditions (Long QT
Syndrome, Hypertrophic Cardiomyopathy, Arrhythmogenic Right Ventricular Cardiomyopathy) through American CLIA-approved laboratories (Genaissance, Harvard Partners Laboratory, Johns
Hopkins respectively) under Ontario Ministry of Health approvals. For non-CLIA approved genes
(DSG2, DSP), research sequencing is performed at the Hospital for Sick Children and a CLIA approval process was initiated through a CETT application in collaboration with Peter Ray.
Over 270 patients have undergone DNA banking, with over 100 positive results. Families are undergoing sequential case expansion to identify genetically positive and confirmed negative family members, with appropriate gene-specific counselling and intervention.
Arrhythmogenic Right Ventricular Cardiomyopathy: Clinical and Research programs
1995-2008: As a pediatric electrophysiologist, high-risk hereditary heart rhythm disorders have been a challenging group of patients to diagnose and treat. I recognized early the potential for
prevention of Sudden Cardiac Arrest or Death (SCA/SCD) through the assessment of patients presenting with warning symptoms (exercise syncope) or a family history of SCA or SCD. With the first report of the genes underlying one of these syndromes (LQT, Keating & Curran, 1994) I initiated a DNA banking program for these conditions. When clinical testing became available in
2003, I arranged genetic testing for all these patients, resulting in SickKids being the site for 70% of provincial diagnoses. Subsequently, genetic testing for hypertrophic cardiomyopathy (HCM) became available and I organized a patient conference on HCM including the option of genetic testing. Clinical genetic testing for Arrhythmogenic Right Ventricular Cardiomyopathy (ARVC) remained limited or unavailable, so I translated our research testing program to SickKids Clinical
Molecular Diagnostics laboratory under a 5 year NIH project called Clinical Education and Test
Translation (CETT) resulting in our lab being listed on genetests.org.
SickKids clinical laboratory continues to perform ARVC gene testing on most Ontario patients, and on New Zealand patients. I succeeded in 2009 in obtaining a $2.5M 5-year grant leading 4 SickKids labs and 3 other UofT labs in gene discovery, functional studies, pathology, protein interactions and drug descovery related to ARVC. This year, I am submitting 2 Genome Canada grants (1 as PI, 1 as co-PI) related to gene discovery in SCA/SCD conditions. I also collaborate with and International
Genome Consortia for gene discovery in LQT and Brugada syndromes. I was a member of 2 panels providing guidelines for Cardiac Genetic testing (Canada, US & Europe, 2011), organized a Toronto cardiac genetics symposium and was invited to speak at the 3 rd Cardiff cardiac genetics symposium.
The development of clinical genetic testing for Arrhythmogenic Right Ventricular Cardiomyopathy has improved the diagnosis of this life-threatening condition, and gene-testing frequently avoids the need for invasive cardiac catheterization and endomyocardial biopsy for the condition, along with the attendant risk of complication. Genetic testing, where positive, allows for cascade screening of family members to identify those requiring further monitoring of their disease and those who can be discharged from follow-up. I have seen and assessed 400+ individuals in cardiac genetics clinic at SickKids for ARVC, and the clinical molecular genetics laboratory has performed ARVC gene testing for an additional 400+ individuals outside of SickKids.
The rapid adoption of genetic testing for other high-risk cardiac conditions (LQT SQT CPVT and
Brugada syndromes and HCM) has similarly aided diagnosis and allowed for cascade genetic testing. The development of Cdn., U.S. and European guidelines for the use of cardiac genetic testing has guided other practitioners in the diagnosis and cascade screening of their patients/families.
Gene discovery work which we are leading for ARVC and contributing to for LQT/Brugada syndromes will allow for identification of remaining genes underlying these conditions, resulting in further success in diagnosis and cascade screening. For some conditions, the specific gene mutation and/or genetic modifiers also contribute to risk, such that a specific risk 'prescription' can be provided to individuals with these conditions.
The developing AV node and its diseases (Congenital Heat Block; CHB and Congenital
Junctional Ectopic Tachycardia; CJET)
1990's to 2008: Congenital heart block (CHB) has always been a strong passion of mine in translational research and clinical care, as it should be a preventable condition. However, little is understood about the specific antibody and its protein/cellular target in the fetal heart. Dr.
Silverman and I identified (some) antibody CHB predictors in neonatal lupus in 1995, and outcomes of mothers in 1996. In 2002, Dr. Jaeggi and I sowed that CHB outcome also related to age at diagnosis (fetus, newborn or child), and in 2006 we showed that we could monitor AV nodal conduction in the fetus (echo and fetal ECG. In 1998 I published an isolated rabbit heart model of
CHB, and in 2005 I published an autoimmunized mouse model including assessment of fetal rhythm. I was a coapplicant on CHB grants with Dr. Silverman in the 1990's and PI on a CIHR grant ifunded in 2005.
2009-2012: I received CIHR grant funding to investigate the AV node target of CHB from 2006-
2010, a 4.2 score on a 2011 app., and have applied to March of Dimes in late 2011 and CIHR in
2012. For the International CHB Symposium in 2010, we published and presented 4 basic science abstracts from my lab, and I gave and invited presentation on the molecular genetics of the AV node. We have now identified the protein target of CHB in the AV node, and if antibodies to a specific area of this protein are found in maternal sera, the fetus is highly likely to get CHB. Thus we have found a marker to specifically identify pregnancies at risk for CHB, and have applied for a patent for this biomarker. We have a publication in submission, and through CIHR and/or internal funding optimize this biomarker with the plan of conducting clinical trials of (steroid) therapy targetted to these high-risk pregnancies in the near future. Idnetification of the fetal target will also guide development of specific therapies for CHB.
We are also performing molecular studies of other AV nodal diseases (Cong. and Postop. Junctional
Tach.)
Identification of a biomarker for CHB will allow current therapies to be targeted only to pregnancies that will be affected by a CHB outcome, rather than treating 100 primry pregnancies to autoimmune mothers to prevent 1 CHB, or treating six subsequent pregnancies to a CHB child to prevent one further CHB. Using our biomarker approach, therapy can begin prior to fetal pathology, providing a higher chance of effective prevention with current steroid therapies. Since we have identified the fetal protein target of maternal antibodies resulting in CHB, we eanticipate that we can make better disease models, which should lead to more specific preventive therapies avoiding
CHB. Our understanding of the molecular biology of the developing AV node, along with whole exome genetic studies, is providing an understanding of congenital JET (a rare but serious hereditary disorder) and postoperative JET, which s the most serious and frequent postoperative arrhythmia in congenital heart surgery.
Guidelines for Cardiovascular Considerations of ADHD Medications
Identification of 'signals' consisting of sudden deaths in young individuals on ADHD medication in the previous decade prompted the FDA to require 'Black Box' warnings and Health Canada to require similar warnings, as well as temporarily suspending one ADHD medication in Canada.
Further evaluation determined that (recognizing the problem of under-reporting in adverse event reporting systems) ADHD drug associated deaths were occurring at below the spontaneous sudden death risk of young individuals, and the 'signal' likely only reflected the very widespread use of these medications. Through this period, ADHD prescribers sought guidance as to how to respond to these warnings, and how to evaluate patients for these purported risks.
Beginning in 2009, I co-led the Canadian guidelines response to these issues, resulting in simultaneous guidelines statements in the Canadian Journal of Adolescent and Child Psychiatry,
Canadian Journal of Cardiology and Pediatric and Child Health in 2009. I also led the European
Network on Hyperkinetic Disorders (Eunethydis) and was lead author on their European Gudelines published in Cardiology in the Young in 2011. I am now the invited Keynote Speaker for the
Research and Scientific Program of the Canadian Academy of Child and Adolescent Psychiatry
(CACAP) 2012 Annual Conference, Sept 30 to Oct 2, Montreal.
FDA and Health Canada warnings for ADHD medication resulted in a large burden on healthcare through the excessive use of cardiovascular testing (ECGs) and cardiac referrals where they would not have otherwise been indicated. Although such warnings may have been in keeping with the application of the 'Precautionary Principle' to the very limited data initially available at the time, subsequent large administrative database studies have demonstrated that there is no measurable increase in Sudden Cardiac Death (SCD) risk in the standard patient treated with ADHD medications. A small subgroup of cardiac patients are at intrinsically increased risk of SCD, although this is likely not modified by ADHD medication. Cardiac patients also have a higher prevalence of
ADHD. Thus, ADHD practitioners and prescribers needed a document to guide their practice with
respect to these cardiavascular considerations in ADHD individuals. I led (Europe) and co-led
(Canada)
It is estimated from administrative databases that patient exposure-years to ADHD medications number in the millions worldwide. Thus, Canadian and European guidelines will have a major impact on guiding appropriate evaluation of these patients.

Cryoablation: Safer Catheter Rx for Arrhythmias in Children
Although we have performed over 1000 catheter ablation procedures in children without any major complication, the broader experience across all centers which perform these procedures for pediatric arrhythmias indicates that atrioventricular (AV) block occurs in 2% of cases. This results in the devastating need for lifelong pacing in these patients. In addition, electrophysiologists may not attempt curative ablation therapy in some patients if they perceive the risk of block is high, and patients often turn down the opportunity of curative ablation therapy when they hear about this possible risk.
A Canadian company, Cryocath Technologies, recently developed a "cold-tip" catheter technology which is capable of cooling the adjacent heart at two levels: a reversible -30 degree lesion or a permanent -70 degree lesion. This allows assessment of the potential effect of a delivered lesion before committing to permanent injury, and should reduce the risk of AV block. I undertook with this company to apply and evaluate this potentially safer technology for catheter ablation of heart rhythm disorders in children. I led our group in obtaining this technology, as well as leading a multicentered pediatric registry.
The program which I created here currently has the largest pediatric experience in catheter cryoablation in the world. We have presented this experience to the North American Society of
Pacing and Electrophysiology (the major international organization for clinical and research cardiac electrophysiologists), the Association of European Pediatric Cardiology and the Charleston
Symposium on International Pediatric Electrophysiology. A manuscript describing the results of our multicentered registry is almost completed.
As a result of this experience and exposure, my program has become a teaching and resource site for this technique in pediatric ablation, with American and international pediatric electrophysiologists coming to visit and observe cases in our clinical laboratory, and frequent calls and correspondence for mentoring in this technique. An example is attached.

Fetal Electrocardiography
I have developed and evaluated non-invasive fetal electrocardiography as a clinical tool for the diagnosis of fetal heart rhythm disorders. The device we use is capable of providing within 10 minutes a fetal electrocardiogram acquired from simple electrodes on the mothers abdomen. I have identified the success rate of this technique in relation to the gestation of the fetus, and have
developed normal values for the fetal PR interval, QRS interval and QT interval in relation to gestational age. I am comparing the values measured with this technique with those measured by both fetal echocardiography and fetal magnetocardiography. The accuracy, reproducibility and utility of fetal electrocardiography in fetal cardiology has been the subject of a current Heart and
Stroke Foundation grant for which I am the Principle Investigator.
Real-Time Asymptomatic Event Monitoring
The intermittent and infrequent nature of arrhythmias in children and adults makes documentation difficult. Current options include 24-hour ambulatory monitoring for asymptomatic events, and longer-term external or implantable recorders for symptomatic events. The prognosis of certain relatively common cardiomyopathies appears to be related to the presence or absence of even infrequent asymptomatic ventricular arrhythmias, but these are difficult to document with current recorders. Indeed, the identification af non-sustained asymptomatic events may alter therapy by supporting the application of primary preventive therapies such as an implantable defibrillator.
The advent of small, sophisticated "palm"-based computing devices should allow for real-time analysis and documentation of asymptomatic arrhythmias over a long period of recording. I am working with a local inventor, Kieth Labelle, of New Products, New Technologies Inc. to refine and assess the accuracy and utility of a real-time asymptomatic event recorder in identifying cardiac arrhythmias.
