All applicants with a Specific Learning Difficulty must submit a full psycho-educational assessment completed by an appropriately qualified psychologist. The report should be dated no older than three years i.e. on or after 1 February 2013. Scores for general ability, cognitive processing and attainment should be reported from tests conducted on or after 1 February 2013. In addition, this Summary Sheet may be attached and should indicate scores for general ability, literacy and mathematical attainments, and any additional disabilities. It is the applicant’s responsibility to ensure that all sections of the Summary Sheet are completed. Remember to keep a photocopy for your personal records.
