POSITION DESCRIPTION
Position Title: Evaluation Specialist
Program: Birth to Three
General Purpose:
Under the direction of the Director of the School Readiness Program to identify children
from 0 to 3 years of age and families in need of direct educational/developmental
services, to provide developmental therapy services and to coordinate service delivery
from among other service and health care providers.
Essential Duties and Responsibilities:
1. Provides direct service to children requiring early intervention services including
educational activities to promote person-social, communication, cognition adaptive and
motor skills development. Educates coaches, parents/caregivers in how to integrate
developmental strategies into daily routines. Promotes health relationships between
eligible children and family members through counseling, demonstrations and providing
relevant materials to those involved. Obtains toys and materials specific to each child’s
developmental need. Maintains log of services delivered to each child and family.
2. Performs developmental evaluations of children and family assessments including
administering developmental tests, assessments of family priorities, resources and
concerns relative to children’s needs, periodic assessments of children and families,
communicating findings of assessments to parents/caregivers, health care providers,
LEA’s and other service providers.
3. Facilitates Individual Family Service Plan (IFSP) meetings with parents and/or other
primary caregivers, health care providers, LEA representatives and others for the purpose
of developing a service plan for each child. Obtains plan approval signatures from
children’s parents or educational parent surrogate. Facilitates 6-month review meetings
or other plan meetings as requested by parents or other team members. Facilitates
transition and PPT meetings to consider educational programs for children reaching age
3. Schedules evaluations/assessments and other meetings related to the
child. Maintains log of all contacts with child/parents and others involved with child’s
care. Obtains all information necessary including parent signatures to process insurance
claims.
4. Makes regular home visits with early childhood educator and paraprofessional to
review child’s program and observe child. Recommends appropriate techniques,
activities and strategies. Consults with coworkers regarding child behavior, personalsocial development issues, family issues and parenting techniques. Participates in the
interviewing of potential employees in the program and provides orientation and ongoing
support to new employees. Provides consultation/training services to other CREC
programs.
Knowledge, Skills and Abilities
Requires MS in Social Work plus 5 years experience in direct early intervention services
to children ages birth to 3 years, or equivalent combination of education and
experience. Must have knowledge of all aspects of early childhood development, various
medical and developmental diagnoses, conditions and treatment methods. Requires
good oral and written communication skills and knowledge of computer use for word
processing, internet and e-mail.
Supervision
No direct supervision. Provides advice and guidance to early childhood educator and
paraprofessional regarding methods/strategies to use in working with children and
families. Participates in interviewing of potential employees, new employee training and
providing ongoing counseling and consultation to coworkers.
Working Conditions
Travels to schools, programs and homes in north central Connecticut averaging
approximately 700-900 miles per month. Homes may be dirty, vermin infested and/or
located in high crime areas. Homes are frequently poorly heated, lighted or
ventilated. Households often have high level of activity and noise. Requires sitting on the
floor with children for extended periods of time, picking them up to facilitate their
development and protecting against physically aggressive children. Requires lifting toy
bags of up to 50 pounds.
