SUMMARY:
The Nurse Practitioner/Physician Assistant will provide a quality outpatient experience for patients at the Hallett Center for
Diabetes and Endocrinology. The Nurse Practitioner/Physician Assistant shall be responsible to the Clinical Director and responsible for overseeing the progress of a specific caseload of patients in the outpatient setting. Act as an educator, consultant, and liaison to healthcare providers caring for the patients across the care continuum.
The Nurse Practitioner/Physician Assistant will provide follow-up care to patients with diabetes and endocrine disorders with attention to medication management for glucose, blood pressure and cholesterol control; other preventive care for diabetic patients; and communication with primary care providers
Will develop expertise in the management of patients with diabetes and other endocrine disorders
Demonstrates a fundamental grounding in nursing/medical theory and practice with mastery of knowledge within a defined content area.
Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.
Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.
Oversees care coordination in acute, home care, and ambulatory care settings as feasible.
Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement.
Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics.
Provides direct care to patients in acute care, ambulatory care and home care settings.
Performs advanced practice skills under the supervision of the Medical Director.
Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee.
Participates in the education, clinical research and mentoring of healthcare providers.
May facilitate patient/family education in area of expertise.
Annually completes mandatory safety infection control, BLS, and ACLS/PALS.
Prescriptive privileges per licensure.
Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
EDUCATION:
Nurse Practitioner
Masters of Science Degree, Graduate of accredited Nurse Practitioner program. Specialty certification required.
Baccalaureate in Nursing
Certified in Basic Cardiac Life Support (BLS).
Current RI licensure.
Physician Assistant
Masters of Science Degree, Graduate of accredited Physician Assistant Program.
Certification
Current RI licensure.
EXPERIENCE PREFERRED (NOT REQUIRED):
Outpatient practice experience
Experience with Diabetes and Endocrine Disorders
American Diabetes Association Certified Diabetes Educator or other equivalent certification
Experience with Electronic Medical Record
