What is the planned dive time? What number of dives are planned and the associated surface interval? What are conditions expected to be like including visibility and water temp? Access to dive site How will the dive site be accessed? For boat diving: What is the name of the boat? Who is the skipper and list their qualifications? What lifesaving appliances are on board? How many people will be on board and what is the maximum number of people permitted on the vessel? List any operating restrictions for the vessel? Attach a copy of the insurance documentation for the vessel Is a VHF radio available and list the call sign of the vessel. The Dive Team Who will be in charge of the dive (Company)? Attach insurance documentation for the diving company Who will act as surface supervisor, list their qualifications including where appropriate diver/ instructor no List members of the dive team and their associated diving qualifications Do all members of the dive team have the appropriate documentation to dive for work purposes? What diving qualifications does the presenter hold? List the diving experience of the presenter including total number of dives and date of last dive Who will be responsible for the presenter when they are in the water? Who is to provide the diving equipment and List details of maintenance regime What specialist equipment is to be used? e.g. rebreathers, full face masks etc Emergency Plan Will oxygen be available at the dive site? List the people who are qualified to administer it. List qualified first aid trained people. Where is the nearest recompression chamber? What is the availability of the chamber? Will it be manned upon arrival and by who? Detail the evacuation route and maximum duration from the dive site to the recompression chamber.
