Kindergarten September 2015 Admissions Process
Welcome Kindergarteners!!!
We are very excited to welcome our newest students. Parents will come to register based on the schedule below which is by your last name. If you cannot come on the scheduled day, there will be additional days scheduled.
*Remember, if you received a placement for Kindergarten in PS 101, the seat is yours/reserved for you, and you do not need to worry about being first to register.
Registration Schedule: 9am-11am daily
Monday, April 20: Last Name begins with A, B or C
Tuesday, April 21: Last Name begins with C, D, E or F
Wednesday, April 22 – Friday, April 24: No Registration due to NYS Math Exam
Monday, April 27: Last Name begins with G, H or I
Tuesday, April 28: Last Name begins with J, K or L
Wednesday, April 29: Last Name begins with M, N, or O
Thursday, April 30: Last Name begins with O, P, Q, or R
Friday, May 1: Last Name begins with S, T or U
Monday, May 4: Last Name begins with V, W, or X
Tuesday, May 5: Last Name begins with Y, Z
Wednesday, May 6: LAST DAY TO REGISTRATION, if you have not already done so!
WHAT YOU NEED TO BRING WHEN YOU ENROLL YOUR CHILD FOR THE
KINDERGARTEN PROCESS:

Original Birth Certificate, Baptismal Certificate, Passport or Visa
Immunization Records:
4-5 doses of Diptheria, Pertussis, Tetanus (DPT)
3-5 doses of Polio
3 doses of Hepatitis B

Proof of Residency (2 items):
Utility bill – gas/electric/water bill only -lease is only acceptable with the following utility bill.
(If utility bill is not in parent’s name then a “Parent Affidavit of Residency” form will be given at that time to be notarized).
VERIFICATION OF RESIDENCY
A. Proof of address must be provided when enrolling a student in school. The following documents may be used as proof of residency:
1. A telephone bill, cable bill, credit card bill, medical insurance card, or driver’s license is not acceptable proof of address.
Proof of address may be verified by any 2 of the following: a. a residential utility bill (gas or electric) i n the resident’s name issued by National Grid (formerly
Keyspan), Con Edison, or the Long Island Power Authority (for the Rockaways) within the prior 60 days.
b. documentation or letter on letterhead from a federal, state, or local government agency, including the
IRS, the City Housing Authority, Human Resources Administration, the Administration for Children’s
Services (ACS), or an ACS subcontractor indicating the resident’s name and address issued within the prior 60 days;

c. an original lease agreement, deed or mortgage statement for the residence; d. a current property tax bill for the residence; e. a water bill for the residence, issued within the past 90 days; f. official payroll documentation from an employer issued within the past 60 days such as a form submitted for tax withholding purposes or a payroll receipt. A letter on the employer’s letterhead is not adequate.
**None of the documents listed above will suffice alone.
2. If a parent is subletting an apartment or home, or if more than one family shares a living space and there is only one leaseholder or homeowner, the parent must present a notarized “Address Affidavit” signed both by the primary leaseholder as well as the parent affirming that the family is residing in this home, and must attach proof of address of the primary leaseholder as specified in Section VII.A.1 (see
Attachment No. 3).
