INFORMATION REGARDING THE ONLINE
STUDENT TUITION COMMITMENT FORM
1.
REGISTRATION DEPOSIT:
Indicate check number and date of your $500.00 deposit check made payable to Bishop Verot Catholic High
School. (If paying by cash please send in an envelope with student name and grade for 2015-2016 school year).

STATE OF FLORIDA SCHOLARSHIPS
If your student is currently receiving either of these scholarships the amount your student is currently receiving
will be filled in this box. The amounts of these scholarships are determined annually by the State of Florida in
July. Parents are responsible for any portion of the tuition and fees not covered by the scholarship. The actual
amount of the scholarship will be deducted from the balance due after the amounts for the scholarship have been
finalized.
3.
PARTICIPATING CATHOLIC ALLOWANCE ($1,000)
This voluntary allowance is provided by Bishop Verot High School for Catholic families that are registered,
participating, and contributing members of a parish in the Diocese of Venice as of the date of registration.
If you require the assistance to help your student attend Bishop Verot, please complete a Parish Card and
submit it to your Parish. The Parish will complete the card and return to Bishop Verot. The Allowance will
not be credited to your account until we receive verification of eligibility from your Parish. Please check
the box appropriate to your situation. Please list your Parish on the form.
If you are eligible for the allowance but wish to voluntarily waive the allowance so that we may use the
funds to help additional students attend Bishop Verot High School for the 2015/2016 school year please
check the box.
4.
NEED BASED TUITION ASSISTANCE AND SCHOLARSHIPS
If you have filed or will file for need based tuition assistance please check the word "Yes." Applications must be
filed with FACTS by March 15, 2015 and Step Up for Students at your earliest convenience. Determination
and notification of need-based tuition assistance will be made in April 2015. Applications submitted after March
15, 2015 will be placed on a waiting list and will not be reviewed until after all timely filed applications have
been reviewed. Please be advised that financial aid funding is limited and applications not filed by the deadline
may result in financial assistance being denied or may result in a reduced award. As financial assistance is
granted on a year by year basis you must file a new application each year. Bishop Verot cannot award any
need based assistance if an application is not timely and properly filed with FACTS and Step Up for
Students.
5.
BALANCE DUE
Notice of the final balance due on tuition and fees will be sent after any financial aid, state scholarships and
FACTS tuition payment plans have been completed, if applicable. Bishop Verot High School attempts to keep our
operating costs and expenses as low as possible. To assist in keeping our costs as low as possible we require that
families either pay the tuition balance in full on/before June 30, 2015 or enroll in FACTS to pay the tuition in
installments. Installment payments begin in July 2015 for the 2015/2016 school year.
PAYMENT OPTIONS

For families that wish to pay the tuition in full, payment is due on/or before June 30, 2015. Credit cards will be
accepted for payment and subject to a 2.5% convenience fee. The school accepts Visa, MasterCard, Discover and
American Express. Credit card payments may be made in person or by phone with FACTS. Full Tuition may also
be paid to FACTS by selecting the “1 payment in full plan” and incur no enrollment fee. Full payment will be
deducted from the account of your choice July 5th or July 20th, 2015.

Installment Payments
Bishop Verot Catholic High School has retained FACTS Tuition Management Systems (FACTS) to assist with
the collection of tuition. All families not paying in full must have an account set up with FACTS. All
payments must be paid to FACTS through your account. Please visit https://online.factsmgt.com/sigin/3V1FB to
enroll in a 10, 11 or 12 month payment plan. All plans begin July 5th or July 20th 2015. Enrollment must be
completed by 6/23/2015 for July 5th disbursements and July 8, 2015 for July 20th. No exceptions.
There is a $36 enrollment fee for the monthly installment payment plan options. This fee will be
automatically processed within 14 days of your finalized agreement. If the tuition is to be paid for or shared
with another party a separate agreement is required for each party.
