JOB DESCRIPTION AND ACTIVITIES:
Duties and responsibilities as SCU- O.R. Staff Nurse.
I.
Administrative Information:
Reports to: Nursing Supervisor/Head Nurse/Chief Nurse
Supervises: Nursing Assistant
II.
General Description of the Job
1. Demonstrate knowledge of general nursing history, principle and
practice of Operating Room Nursing.
2. Interprets and upholds policies and procedures as determined by the
administrative body.
3. Ensures availability of supplies and equipments anytime. Informs
appropriate personnel when supplies are needed.
4. Assists in orientation of professional and non-professional personnel and
nursing students to the basic procedures and techniques of the assigned
areas.
5. Utilizes knowledge of medical and surgical aseptic techniques, rules and
regulations while acting as a circulating nurse, scrub nurse or teacher.
6. Ensures quality- nursing care through the maintenance of instruments,
techniques, equipments, supplies and cleaning in the assigned areas.
7. Maintains good interpersonal relationship with other members of the
working team.
8. Identifies and studies nursing care problems and assists in their solutions.
9. Such other duties from time to time as may be prescribed by the proper
authorities.
SCRUB NURSE
1. Assist the surgeon and assistant surgeon by providing the sterile
instrument and supplies needed and at the same time, anticipate plans
and responds to surgeons needs by constantly observing the sterile
field.
2. Maintains the integrity, safety and efficiency of the sterile field
throughout the procedure.
3. Together with the circulating nurse do the initial sponge, instrument
and needle count prior to the surgery. Likewise, coordinates with the
circulating nurse when about to close the operative site for the sponge,
instrument and needle count.
4. Assist the surgeons and assistants in gloving and gowning.
5. Assist in the transfer of patient from the Operating Room table to the
Recovery Room.
6. Takes charge in the after care of the instruments as well as the
equipments used in the procedure and overall orderliness and
cleanliness/disinfections inside the Operating Room.
7. Responsible for the specimen to be forward to the laboratory.
8. Prepares the room for the next cases.
CIRCULATING NURSE
1. Takes charge of the activities of the Operating Room outside the sterile
filed by creating and maintaining a safe and comfortable environment
for each patient. Makes sure no break is made in the practice of aseptic
technique.
2. Provides assistance to any member of the Operating Room team such
as seeing that the team is supplied with every item necessary and
knowing all supplies, instruments and equipments.
3. Before the operation does the routine checklist for each patient
ensuring quality pre-operative nursing care. Receives the patient with
proper endorsement from the ward nurse. Likewise attends to proper
endorsement of post-operative patients to the Recovery Room nurse.
4. Attends to the records and documentation.
5. Together with the scrub nurse, attends to the initial sponge, instrument
and needle count before the start of operation as well as before wound
closure. Informs the surgeon of the completeness or incompleteness of
the count.
6. Checks the cleanliness, orderliness and disinfections of the room used.
Helps prepare the room for the next case.
7. Endorses the list of chargeable items to clerk.
8. Help in washing and packing of the instruments used.
Duties and responsibilities as GNU Staff Nurse.
1. Planning of Nursing Care
a. Identifies needs and problems of patients through interview and
clinical assessment.
b. Prepares the nursing care assignment in accordance with the
education, experience and capabilities of the individual team
members.
c. Coordinates with the Clinical Instructors the assignment of nursing
and midwifery students.
2. Assists the Physician in their medical care of patients.
a. Informs the Attending Physician, Residents/ Interns in- charge of
patient admission and other referrals.
b. Make rounds with the Attending Physician to familiarize himself
with the patient’s problems and the plan of the management.
c. Carries out written orders of Physician.
d. Follows up all laboratory and diagnostic test results, incorporates
copy of the results in the patient’s charts, and relay such results to
Attending Physician/ Residents.
e. Assists the Physician in performing certain bedside procedures
such as thoracentesis, lumbar tap, chemotherapy and paracentesis.
f. Coordinates with the members of the nursing staff the monitoring
and accurate recording of IVT infusions, blood transfusions, I & O
and vital signs.
g. Supervises in the administration of both oral and parenteral
medications with corresponding documentation.
h. Checks for completeness of E- Cart contents during his shift and
reports to Head Nurse and deficiencies or discrepancies.
i. Facilitates patient’s discharge.
3. Communicates with other members of the health care team,
developments in the patient’s clinical course.
a. Transcribes all Physicians’ orders into kardex, medication and
treatment sheet, and standing order sheet. Makes the
corresponding medicines/ treatment cards and endorses this to the
concerned staff.
b. Refers to the Resident Physician on duty and the Attending
Physician any patient related problems.
c. Receives and gives complete shift endorsement.
d. Sees to it that patient charts and all appended legal documents are
properly accomplished and forwarded to Medical Records Section
upon discharge.
e. Reports to the Head Nurse or Nurse Supervisor on duty any
significant incident that occurred in the unit within the shift.
4. Evaluates effectiveness of nursing care in relation to goals.
a. Sees to it that all patients in his unit receive the necessary nursing
care.
b. Checks on the performance and completion of assigned duties and
responsibilities.
c. Assists in the orientation, education and evaluation of personnel
and students.
5. Attends monthly Nursing Service meetings and upgrades knowledge
and nursing skills through participation in the In- Service Training
Program.
6. As Educator:
a. Orients the student nurse in the personnel, philosophies and
objectives of the hospital.
b. Orients the student nurse in the different set- up and department of
the hospital
c. Teaches and demonstrate to the student all the basic principles of
care to patients with medical as well as surgical conditions.
7. Others:
a. Acts as Head Nurse when delegated to do so.
b. Accepts such other duties as may from time to time be prescribed
by proper authorities.
