2015 FOOD FRENZY
RESULTS REPORTING PACKET
Please deliver this packet with all monetary donations and forms on Tuesday, July 28 between
9am-12pm to either of the following location:


Davis Wright Tremaine, 1201 Third Avenue, 22nd Floor, Seattle
Foundation Bank, 1110 112th Ave NE, 2nd Floor, West, Bellevue
Or mail/deliver to Food Lifeline, Attn: Food Frenzy, 1702 NE 150 th St, Shoreline, WA 98155.
Packets must be received before noon to Davis Wright Tremaine or Foundation Bank or before
5pm to Food Lifeline on July 29 to be included in the competition. Please call Food Lifeline with
any questions, 206-545-6600 x3624.
All donors who provide a complete address will be mailed an acknowledgment letter at the
address indicated, unless the following box is checked:
 Please send one packet of all employee acknowledgment letters to our office.

VOLUNTEER HOURS
Only volunteer hours accrued between July 30, 2014 and July 28, 2015 will count
toward your company’s Food Frenzy 2015 points. We trust the Food Frenzy coordinator
from each participating firm to honestly and accurately track their employees' volunteer
hours. Food Lifeline is able to verify hours as necessary.

donations, Food Lifeline counts on you to drop off your food at one of Food Lifeline’s
warehouses or hosted drop-off locations.
