This booklet has been designed to provide you with the necessary information required to make an informed choice on your care provider and/or assist you in making your stay with us more comfortable. It will also meet the requirements set out in the ‘National Quality Standards for Residential Care Settings for Older People in Ireland’ laid down by the Health Information and Quality Authority. These standards are enacted by the Health Act 2007 (Care and Welfare of Residents in Designated Centers for Older People) Regulations 2009. This booklet will provide an introduction to the care and services provided within Oakland Nursing Home. Further information and more detailed explanations of the governance and management of the nursing home can be facilitated by making an appointment with Management, at a time suitable to you. To successfully achieve this we believe this requires a multitalented approach to care Our objective is to ensure the continued delivery of high quality consistent care to all residents. This will be achieved by: Adherence to best practice policies Continued recruitment and most importantly the retention of highly skilled staff Continual training of staff Constant performance review of the home Being receptive to feedback from residents Integrated Residents Records Individualised, Specific Care Plans Continued development and implementation of Best Practice Standards In addition to the Quality of Care, Oaklands Nursing Home also pays particular attention to: 2 Tailor made activity and care plans depending on the ability and requirement of our residents. This is recorded in the Residents Life Story record, highlighting specific preferences and requirements requested by each resident. Interesting and varied menus - quality food is such an important aspect of life within the home. Our cooks take a real interest in their dishes and are constantly changing and improving depending on individuals requirements. Involvement with the local community is vital - Oaklands Nursing Home welcomes visits especially from local musicians, and singers. Additionally, we have voluntary workers who attend the home daily to carry out various activities Each resident will be identified with 2 Key workers upon admission, a Nurse and a Health Care Assistant, who will oversee your individual care plan on a weekly basis. Oaklands Nursing Home Residents Charter of Rights Each resident in a nursing home has the right to: Be accepted as an individual and have his or her preferences taken into account Be addressed in a form he or she is happy with Select and maintain family, social and personal relationships with any other person, both within and outside the nursing home Protection from harm and exploitation Receive a contract outlining the rights and obligations of both the nursing home and the resident 24 Hour Quality care which is appropriate to your needs Ensure residents and where necessary their families are informed of all services that may be relevant to their needs regardless of their immediate availability Full information about your own state of health and about available treatments Maintain control over, and continue to make decisions about, the personal aspects of his or her daily life, financial affairs and possessions Be consulted on, and to choose to have an input into, decisions about your living arrangements in the home on a daily basis Exercise all of their civil and natural rights and to have access to services and activities which are generally available in the community Personal privacy Live without being obliged to feel grateful to those providing his or her care and accommodation Live in a safe, secure and homelike environment, and to move freely both within and outside the nursing home without undue/unnecessary restrictions Maintain your personal independence, which includes a recognition of personal responsibility for your own actions or choices, including those within which there is a degree of personal risk Take responsibility for your own personal affairs and to undertake daily living tasks of which they are capable Be treated with dignity and respect 3 Oaklands Nursing Home Description of care provided: Oaklands Nursing Home Nursing Home can accommodate up to 38 residents. We employ a professional staff consisting of a management team, registered nurses, care assistants, maintenance, laundry, housekeeping, administration and catering staff. We provide care for all levels of dependency, and all individual needs, providing 24 hours care delivery. Descriptions of dependency levels as used by HIQA in their “Annual Return for Providers of Designated Centres: Residential Care Centres for Older People” are as follows: Low dependency: This category refers to people who need some support in the community and the more independent residents in residential accommodation who require little nursing care. They are usually independently mobile but may use a walking stick and have difficulty managing stairs. Medium Dependency: Person whose independence is impaired to the extent that he/she requires residential care because the appropriate support and nursing care required by the person cannot be provided by the community. Mobility is impaired to the extent that the person requires supervision or a walking aid. High Dependency: Independence is impaired to the extent that the person requires residential care but is not bed bound. The person may have a combination of physical and mental disabilities, may be confused at times and be incontinent. He/she may require a walking aid and physical assistance to walk. Maximum Dependency: Person whose independence is impaired to the extent that he/she requires nursing care. The person is likely to be bed bound, requires assistance with all aspects of physical care and may be ambulant but confused, disturbed and incontinent. Our organizational chart is on display in Reception area, please feel free to discuss with staff. External professionals attend the home on a regular basis to ensure residents needs are met, Dietician, Physio, Psychiatry, Tissue Viability Nurse, Opticians, Public Health Nurses, Geriatrician, Pharmacist, Consultants Occupancy/ Resident profile: Oaklands Nursing Home can accommodate up to 38 residents at the moment, but is currently under renovations to increase this to 51 residents in 2011. We accommodate both male and female residents of all ages with the following care needs: General Residential Care, Respite care, Intermittent Respite Care, Post Operative Convalescent Care, and General Medical Convalescent Care. Admission Criteria: Admissions to Oaklands Nursing Home can be arranged by Nurse Liaison Officers within the Hospitals, district Nurses, the G.Ps, family members or the resident. Family members or 5 Oaklands Nursing Home individual potential residents are welcome to view the nursing home prior to admission, upon scheduling an appointment with the person in charge. The Person In charge will also meet with you externally if you wish to arrange and finalise all aspects of Admission. We will ensure that we will do everything possible to ensure your transition into the Nursing Home will be as comfortable as possible, for both you and your family. You are welcome to bring any personal items you so wish, in order to transform your bedroom into your own familiar, tranquil space. The aim is that Oaklands will become your ‘Home from Home’. Arrangements for Visiting Potential Residents We understand that the decision to move into long-term care can be a stressful time. At Oaklands Nursing Home we want to make your transition as smooth as possible. Our Nurse in Charge or Administrator will be happy to meet with you and your family to give you a tour of the building and discuss any personal needs you may have. In our experience the more information we obtain with regard to the prospective resident prior to an admission in relation to health, medication, personality, likes and dislikes the smoother the transition is to the home. In order to ensure you receive our uninterrupted attention we would ask that you kindly schedule an appointment in the first instance. Existing Residents We operate an open visiting policy within Oaklands Nursing Home however we do ask that the resident’s mealtimes be respected so visitors are not permitted into the dining area at mealtimes. In order to protect our residents we ask that all visitors sign in and out on entering and leaving and partake in precautionary infection control measures by using hand sanitizers provided. Oaklands Nursing Home reserves the right to impose restrictions on visiting arrangements where the visit, or time of visit is deemed to pose a risk, or if visitors is suffering from infectious illness deemed to pose a risk to resident, or where the resident themselves requests restrictions. If visitors are unfamiliar with the home they can ask any member of staff for assistance at any time. 6 Oaklands Nursing Home For security reasons, the main door to the Nursing home is locked, and will be opened by staff once the door bell is rung. All visitors are requested to sign in and out, we ask for your understanding on this matter. Accommodation Provided and Environmental Facilities: Oaklands Nursing Home is made up of all ensuite bedrooms, all wheelchair accessible, 7 Double rooms, 24 Single rooms. Minimum dimensions are 9.3m² single and minimum dimensions are 14.8m² for a double. While renovations to building are on-going, there are 1 additional resident’s toilets of which is wheelchair assessable, 1 staff room, smoking room, a kitchen, a dining area, 1 laundry, 1 day/rest rooms, a lobby, a hygiene room, 1 offices and 1 nurses station. Resources will be improved once renovation complete, providing new dining room, sitting room, smoking room, prayer room, assisted Baths, Clinical room, store room, laundry and Housekeeping room. There is 1 secure outdoor enclosed courtyard with seating areas for residents to enjoy weather permitting Confidence in our ability/ Complaints and Advocacy: Oaklands Nursing Home strives to provide a high quality service to all residents. There is a structured process for receiving and acting upon comments, compliments and complaints. This process is open, honest and strictly confidential and we would urge you to direct your comments to the Person in Charge in the first instance. We encourage family participation in your care and therefore welcome comments from anyone acting on your behalf. We will of course check that they have your permission. We welcome your views no matter how big or small as it can only enhance better quality of care for our residents. 7 Oaklands Nursing Home If you are unsatisfied with our response you may seek assistance from a recognised external advocacy group as displayed at the end of this booklet. External Facilities/ Activities: Oaklands Nursing Home is situated in the tranquil country side just 4 miles from Listowel Heritage Town. The Nursing Home is therefore serviced by nearby hotels, restaurants, public houses, shops, library, Siámsa Tíre community hall, and local Alzheimer’s day care centre, GP surgeries, South Doc. Oaklands Nursing Home will assist you to participate in external activities when required. Additional costs may be incurred for these services which will be explained and agreed prior. 8 Oaklands Nursing Home Safety: Your safety is of paramount importance to us. If you need assistance please ask a member of staff or alternatively use your call bell to seek assistance. Fire: We have installed the very latest fire alarm system by Apex fire Ltd; routine fire alarm testing will be carried out. You will be informed of this upon admission. Other Emergencies: If you discover other scenarios/ circumstances which pose a risk to residents/ staff, please inform your nearest staff member immediately. We have a trained Emergency Response Team and are always available on each shift. Privacy and Dignity: We would like you to think of Oaklands Nursing Home as your home from home. Our staff will do their utmost to protect your privacy and dignity by knocking before entering your room asking your permission prior to any personal/ nursing interventions asking your permission for staff undergoing training and development, members of the opposite sex or others to be involved in your care Assistance with daily activities, and involvement within the home. If you feel your privacy and dignity is being compromised then please inform a member of staff that you feel comfortable with. 9 Oaklands Nursing Home Individualised Care plan: Your care plan will be developed with your participation within 48 hours of admission. This will be individualised to set out your personal care needs and will provide direction to staff members caring for you. A review of your care plan will be prompted following your feedback, any changes in your personal needs/ circumstances and will be updated no less frequently than at three-monthly intervals. To ensure we have your full participation in this process we will communicate to you when a review is required and will then set a mutually convenient time to complete the review process. Contract of Care: By agreeing to take up residency within Oaklands Nursing Home you will have signed a contract of care which ensures that you have a legally binding assurance of high quality care standards and that we have an acknowledgement of your commitment to our terms and conditions which are as follows: Management will meet with you to discuss the contract and assist you with Fair Deal applications for funding as much as possible. Under the Fair Deal scheme some items required for care, are not covered by funding, and a copy of these will be listed within your contract. Fee’s are payable every 28 days in a manner which is suitable to you. Individual needs and circumstances will be dealt with in a proficient and confidential manner by management. There are a number of ways in which you can share your views/ participate in the consultation process: You may also nominate a family member, friend or advocate acting on your behalf. We will of course check that they have your permission. We take all comments seriously and will aim to respond to your query as soon as possible. If you are not satisfied with your initial response then you may contact our Director of Nursing Complaints Procedure Oaklands Nursing Home welcomes all comments, complaints and compliments. A complaint may be verbally reported to Person in Charge, or in written format on a complaints form Informal Complaint 1. Speak informally to any member of staff and outline your concerns. 2. Should your concerns not be resolved to your satisfaction speak with the Director of Care. 3. If unsatisfied with this outcome, speak with the Registered Providers, Michael and Mary O’Donoghue. Formal Complaint 1. If a resident or Relative wishes to make a formal complaint they must do so in writing to the Director of Care or to the Registered Provider 2. The written communication should include the following information: An outline of the complaint Date, Time and Place (if appropriate) of the incident The name of person(s) involved in or witness to the incident (if appropriate) 3. Complaints will be acknowledged upon receipt within three working days. 12 Oaklands Nursing Home 4. A full response will be given within 1 working week. If a delay occurs the complainant will be advised in writing of the reason for the delay. If you feel your complaint remains unresolved you should forward your complaint in writing to the Nursing Home Inspectorate.
