Goals
1. To be able to successfully give a resident a bed bath by March 17th, 2015, while
maintaining their dignity and keeping them comfortable despite their vulnerability. I will
respect the privacy of the resident and do my best to ensure an effective and positive
encounter
2. I will be able to lift and transfer residents without any injury to others, the resident or
myself directly. This will be done comfortably, with confidence and efficiency, without
too much guidance and with ease
Activities, Necessary Resources and Timeframes to Achieve My Goal?

Review the textbook material’s directions and steps to provide the resident with a proper
bed bath. I will complete this before entering into the clinical setting, as needed

Locate a peer-reviewed, scholarly article on the proper directions to perform a bed bath.
This article must also contain directions on how to maintain the privacy and dignity of
the resident in an efficient manner

Similar to the bed bath goal, I will review the textbook on necessary information on
proper technique for lifts and transfers of a resident

I will find a peer-reviewed, scholarly article containing the different types of lifts and
retain all knowledge on the proper techniques required, as described in the articles
Evaluation

To know that I have achieved my first goal, I will successfully have given the resident a
bed bath where they remain comfortable throughout the entire process. I will have
completed and utilized all directions provided by the textbook and scholarly article
accurately

If I have performed the bed bath well, and maintained their privacy and dignity in an
effective matter, the resident will, most likely, be content after the bath instead of
conserved and potentially angry or upset. I will complete this goal by week 9 of clinical

To ensure that I have completed my second goal, I will have lifted/transferred several
residents without the guidance of a PSW and no injuries or accidents will have occurred

This lifts/transfers will have been completed in coherence to the proper techniques
outlined in the textbook and/or scholarly journal that was selected and will be completed
by week 7 of clinical

Goals
1. During my experience in a residential long-term care setting, I will develop a therapeutic
nurse-client relationship with my resident. This will be done using 2 non-verbal forms of
communication during the daily care routine.
2. As a student, I will become knowledgeable with the social, biological and cognitive
aspects of an aging older adult. I will also educate myself with the physical changes and
potential risks that could arise due to the increasing frailty of the resident’s body


To develop a therapeutic nurse-client relationship with my resident by the end of the
clinical placement, I will review the components and effective ways of developing this
relationship from NURS1000’s content

I will also review textbook content and a scholarly journal on the formation of therapeutic
nurse-client relationships

To ensure the knowledge of the social, biological and cognitive aspects of an aging older
adult, I will review the lecture notes from NURS1002 and will find two scholarly
journals pertinent to this information

I will have this knowledge reviewed and known by week 6 of clinical placement to be
able to apply it to the following weeks
Evaluation

To know that I have achieved my first goal, there will be comfortable encounters at all
times between me and my resident, with conversation that flows easily and a formation of
trust

The five components of a therapeutic nurse-client relationship, trust, respect, professional
intimacy, empathy and power (CNO, 2013), will all be demonstrated within the
relationship between myself and the resident by the end of clinical

To ensure that I have completed the second goal in this category, I will have reviewed all
material pertinent to the changes that the elderly experience and I will be able to
recognize it when I observe the residents

I will be able to suggest ways in which to lift/transfer, bathe, etc. the resident in
accordance with their changing bodies and the potential risks that are associated
