Annual Dental Savings Plan
As a benefit to our patients without dental insurance, our office has created an
Annual Dental Savings Plan. In addition to a Prevention Package, this plan also
provides you with a 10% savings on all dental services. We hope this plan will
allow you to enjoy optimal health.
Prevention Package
• 2 Routine Prophylaxis (cleanings)
• Bitewing x-rays per year
• Oral exam, including oral cancer screening
• Fluoride treatment
Fee: $397 ($445 value!)
Prevention Package (children 13 and under)
• 2 Routine Prophylaxis (cleanings)
• Bitewing x-rays
• Oral exam
• Fluoride treatment
• Oral hygiene instruction
Fee: $357 ($467 value!)
Periodontal Maintenance Package
• 3 Periodontal Therapy appointments
• Oral exam
• Bitewing x-rays
• Fluoride treatment
Fee: $567 ($633 value)
***All other dental services are provided at a savings of 10%!
Guidelines:
1. Savings plan only eligible at the office of Dr. Michael Long.
2. All fees due and payable at time of registration.
3. For 4 or more family members, 6-month financing is available. Family members are
defined as spouse and dependent children under 26 years old. We will auto-bill
your credit card monthly.

5. Prevention Package is for a one year term, starting at registration date.
6. All additional charges are due the day of service.
7. This is a dental savings plan, not a dental insurance plan.
