Reporting Template High School Health and Fitness Assessments We designed this worksheet form for optional use only as an efficient way to capture data about Health and Fitness assessments. State law requires school districts to report whether or not the district administered assessments in the areas of Social Studies, The Arts, Health, and Fitness at the elementary, middle, and high school levels, per RCW 28A.230.095. Please note that specific information for individual teachers and schools will not be collected by OSPI. Although not compulsory on the report, we also hope to gather information about what assessments or if other strategies are used by school districts to measure student learning in these subjects. If you decide to use this form, please share it with the person in your district who handles reporting for OSPI. To complete this worksheet: 1. Fill in the total (unduplicated) number of students who completed one or more assessment for Health and Fitness. 2.
