THE NORWEGIAN BUHUND
The health survey consists of 3 parts. Check your information before you proceed from part 1
to part 2 and from 2 to 3. You will not be able to backtrack to previous parts without deleting
all information.
Certain illnesses are very infrequent in the race. If you are unfamiliar with the term of an
illness, just skip the question. Whoever has had such a rare disease, would be familiar with the
term.
When finally you press ”Send svar”, you will see an empty form. This means that the
registration is complete. In case you have additional dogs, just carry on with dog no. 2. If not
we thank you for your contribution!
Part 1 of 3 - GENERAL INFORMATION
This part is confidential and remains with the Norwegian Veterinary University! The
information on the dog is necessary to check as to the correctness of race and to eliminate the
possibility of the same dog participating in the survey more than once. This information also
enables the Norwegian Veterinary University to contact the owner so as to eradicate possible
