2015 Scholarship Application Cover Sheet
Ralph W. & Emza E. Thurston Basketball Scholarship
This scholarship was established to recognize and reward outstanding male or female basketball players at
Alexandria‐Monroe High School. Applications will be accepted only from senior basketball players at Alexandria‐
Monroe High School. Financial need is a very important consideration of this award.
The Ralph W. & Emza E. Thurston Basketball Scholarship is open to applicants who meet the following
eligibility requirements:
 Have been a Madison County Resident for 12 months prior to application.
 Are graduating seniors attending Alexandria-Monroe High School.
 Played basketball for AMHS as seniors.
 Have maintained a 3.0 GPA
 Plan to attend a 2 or 4 year college, university or trade school.
Eligible applicants for this scholarship will be evaluated on the following criteria:
 Athletic performance and dedication.
 GPA, academic rigor, and SAT/ACT performance.
 Demonstration of Financial Need
o College Cost Estimator
o Compelling extenuating circumstances
Required attachments for the Ralph W. & Emza E. Thurston Basketball Scholarship:
 Essay- In no more than 250 words, write an essay on why you believe you should be selected for this
scholarship.

Applications are due to the offices of the Madison County Community Foundation by 4pm on Thursday, February 26, 2015.
Applications may be hand delivered or delivered via postal service. Postmarks are not accepted as proof of delivery. Complete the
2015 General Scholarship Application in full, including all required attachments, and submit the original, clearly
marked as such on this Cover Page, and staple. Additionally, submit seven stapled copies of the completed Application, Cover Page
and appropriate attachments (as listed above) for review by the Scholarship Committee. You are also required to complete
the College Cost Estimator by 4pm on Thursday, February 26, 2015. Instructions are included on the following page
for the College Cost Estimator. Incomplete or late applications will necessarily be disqualified and will not be reviewed. Please

Thanks to a grant from the Madison County Community Foundation, we are
pleased to bring you the free, confidential, College Costs Estimator service. Since
you are applying for one or more need-based scholarships through the Madison
County Community Foundation, completion of the College Costs Estimator is
required. Below are instructions to help you complete the form online:
Step 1. Connect to www.indianacollegecosts.org using your favorite Internet
browser.
Step 2. Click on the yellow button which says “Click here to Begin!” Now click on
“Estimate your college costs” at the top of the page.
Step 3. Gather the information necessary to complete the form, read the confidentiality
note, and if you agree check the box and click “Start New Estimator.” You will then be sent
to a “Demographics” page, the first of six pages that make up the form.
Step 4. Be sure to check “yes” to the question in the yellow box that asks if you are
applying for scholarships. Also be sure to check which scholarship(s) you are applying
for. Once you have completed the page, you may click “Create Account” to create a new
account, or “Login” if you already have a username and password
through the site.
Step 5. When finished entering data, be sure to check the summary page to make sure
everything was entered correctly then click “Submit your Estimator.” Here you can
choose the delivery method for your report to be returned to you. You will also sign a
release form electronically. Once you have signed the release form, click “Submit my
Estimator.”
Please submit ASAP, but no later than the deadline
stated in your scholarship application.
**You will be able to view your results at this time, but after verification and/or follow- up
by our staff, the final result may vary.
Step 6. Your College Costs Estimator report will be mailed or emailed to you based on the
option you chose. You may call our office to discuss your report with a consultant at (877)
