RD will be able to locate resources, information and
equipment necessary to assess nutritional status
and needs of organ transplant candidates.
Demonstrate ability to locate staff and information
for referred patients as well as process for making
referrals for patients who need additional
interventions in order to achieve nutrition goals.
Demonstrates ability to coordinate with
multidisciplinary team the evaluation process for
potential transplant candidate, including but not
limited to:
- Nutritional status
- Ability to meet nutritional
needs/recommendations
Demonstrates ability to document nutrition
consults in the EMR
Ability to provide and maintain comprehensive
transplant nutrition and intestinal rehab nutrition

 Rounds on inpatients as part of multidisciplinary

 Participates in transplant selection committee
 Review patient records and provides
recommendations as needed
 Provides nutritional information as requested to
other health care providers in both the inpatient
and outpatient setting
