Cannot perform this skill PERFORMANCE CRITERIA Standard RD will be able to locate resources, information and equipment necessary to assess nutritional status and needs of organ transplant candidates. Demonstrate ability to locate staff and information for referred patients as well as process for making referrals for patients who need additional interventions in order to achieve nutrition goals. Demonstrates ability to coordinate with multidisciplinary team the evaluation process for potential transplant candidate, including but not limited to: - Nutritional status - Ability to meet nutritional needs/recommendations Demonstrates ability to document nutrition consults in the EMR Ability to provide and maintain comprehensive transplant nutrition and intestinal rehab nutrition documentation and make appropriate referrals to Level of Performance Assessment Method Comments specialists. Implement changes in nutritional/parenteral nutrition therapy as ordered by provider. Monitor patient progress between clinic visits and share pertinent information with team Staff Education As needed, routine and systematic teaching of medical, nursing, and other personnel regarding nutritional needs of the solid organ transplant recipient and the intestinal rehabilitation patient.
