IEP Progress Monitoring Directions What is progress monitoring? Progress monitoring is the ongoing process of collecting and analyzing data to determine student progress. Collecting ongoing progress monitoring data and informing parents of student progress toward annual IEP goals is a legal requirement. (707 KAR 1:320) Who is responsible for progress monitoring? The IEP case manager is responsible for compiling progress monitoring data and reporting progress towards annual IEP goals as often as the school reports progress for all students. How often is progress monitoring data collected? For academic goals: Progress monitoring must take place at a minimum of once every two weeks. For behavior goals: Progress monitoring must take place at a minimum of once a week. What are the steps for completing progress monitoring? 1) Choose an appropriate progress monitoring tool for each objective. Some examples of progress monitoring tools: work samples, informal assessments (probes), anecdotal notes, behavior chart. Resources for progress monitoring tools: www.easyCBM.com www.interventioncentral.com www.studentprogress.org 2) Use selected progress monitoring tool to collect data. 3) Record data on the Hardin County IEP Progress Monitoring Summary form. ***see directions below*** 4) Store all progress monitoring tools used to collect data in a folder with summary form. A copy of the summary form should be placed in the student’s blue binder with the IEP. 5) Send a copy of the Hardin County IEP Progress Monitoring Summary form to the parent when progress is reported for all students. Send home the IEP Progress Monitoring Summary form with graph with report cards (at minimum). ***Directions for Completion of Summary form*** Use a separate summary sheet for each IEP annual goal. For each annual IEP goal, complete the following steps: Complete the student’s demographic information. Record the name of the IEP case manager and the beginning and end dates of the IEP on each summary sheet. Record the annual goal in the designated text box. This can be copied and pasted from a Word document or Infinite Campus. Record each objective in a designated text box. This can be copied and pasted from a Word document or Infinite Campus. Under each objective there are two rows of cells. **In the top row, record the week ending date for each week of school, beginning with the student’s IEP beginning date. Do not include breaks (Fall, Winter, Spring, Summer). In the bottom row, record numerical progress monitoring data. Record data for behavior goals at least once a week. Record data for academic goals at least once every two weeks. Sample Objective 1: When given a list of sight word vocabulary to aid in comprehending text that Maria has been exposed to during classroom instruction, Maria will orally read the words with 80% accuracy.
