Now Hiring Director, Accountable Care Professional Pediatric Home Care, Inc. (PPHC) is currently recruiting for a Director of Accountable Care. Professional Pediatric Home Care (PPHC) is Colorado’s leading provider of pediatric home-based, private duty nursing (PDN) and therapy, established in 1979 to provide highly skilled pediatric nursing, occupational, physical and speech therapy care for children in combination and coordination with other primary care and specialty care providers. Our pediatric staff specializes in delivering comprehensive, collaborative,multidisciplinary and family centered care for infants, children and adolescents with a variety of diagnoses and needs from acute nursing care to short-term therapies. PPHC is licensed by the State of Colorado, ACHC accredited and Medicare/Medicaid certified. The CE/QD designs and conducts orientation and educational programs for clinical staff. The Educator coordinates orientation programs for new staff members, develops programs to keep existing staff up-to-date on clinical practices, and acts as general resource for nursing personnel. The CE/QD is also responsible for the Quality Management Program (QMP), to include interaction with key clinical referral sources to ensure alignment of organizational quality and clinical expectations. Assists the Agency Administrator in the coordination of any on-site audits/investigations by various external reviewers (i.e. CDPHE, ACHC, CMS). QUALIFICATIONS: Requires a Bachelor’s degree in Nursing (Master’s preferred). 2. At least 3-5 years of healthcare experience, Pediatric specialty preferred. 3. Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing. 4. Ability to effectively train new nurses, nurses new to pediatrics and/or home care and experienced nurses who are new to home care. 5. Accepts the Agency Mission, Vision and goals. RESPONSIBILITIES: Clinical Educator 1. 2. 3. 4. 5. 6. 7. 8. 9. 10. 11. 12. Educates and develops pediatric patient care skills for nurses along the continuum. Provides Orientation Program for new clinical staff. Develops educational programs based on the needs of clinical staff, in coordination with the Nurse Case Managers. Provides educational support for policies and procedures which meet CMS guidelines, ACHC requirements, and federal, state and local laws. Creates new policies and procedures as needed, based on clinician input and quality reviews and current practice standards. Educates all clinical staff regarding the importance of complete and pertinent documentation in order to ensure safe patient care and maintain ACHC standards. Ensures the proper education/orientation for each nurse specific to the client’s required interventions Documents the educational progression of care providers (Orientation, Annual Education etc). Works closely with referral sources to develop the New Graduate Training Program, in order to provide evidence-based training and to support the educational vision and clinical proficiency expectations of the referral sources. Provides general clinical support to all care providers. Provides training/support for electronic documentation system Shares ‘on-call’ responsibilities. RESPONSIBILITIES: Quality Director 1. Responsible for the implementation of the Quality Management Program, as per State and Federal regulations. 2. Performs data collection, analysis and reporting for quality, risk and service-related activities (i.e. complaints, falls, medication events, etc.) Continuously improves processes of patient care/services as well as outcomes of care. Works with managers, supervisors and front line staff to develop action plans to improve outcomes, including the development of specific educational programs based on problem identification/needs. Reviews/analyzes all Adverse Event forms and uses a systematic approach to problem identification and resolution.
