contains the gateway for Nurses to select,
create, view and approve narratives. These
narratives are based on, and approved by,
the Oncology Nursing Society (ONS).
Nursing Home Page

Double click on selected Patient. (It will bring up the Patient Navigator)

Initial Nurse Assessment

From the Nursing Home Page, click Create New Narrative.
Click Initial Nurse Assessment.
Step through the tabs, answering as many questions as appropriate.
a. Answers from ONCOintake will populate the form.
b. These answers can be edited as needed.
Click Save for later review or click Save & Record to lock the document and sign your
name.
The majority of your answers can flow into the physicians’ Initial Evaluation if your center
is setup to cascade, but only if the Nursing Initial Assessment is saved prior to the
Physician’s Initial Evaluation.
Nursing - Radiation Therapy Patient Record (PCR) (Nursing Weekly

Click Radiation Therapy Patient Care Record.
Select the appropriate form.
Step through the tabs, answering as many questions as appropriate.

center is setup to cascade.

Nurse Checks

Nurse Check narratives are useful for documentation of phone calls, short visits, lab
documentation and situations other than the Initial Note, Weekly Note or Follow-Up Note
From the Nursing Home Page, click Create New Narrative.
Select Nurse Check (CPT 99211 No Billing) to perform a short note but not send a CPT
Code to billing.

Select Nurse Check (Bills CPT 99211) to perform a short note and send CPT Code 99211
to billing. This requires the physician to review and sign the note.

Click Teaching and Instructions.
Select the appropriate form.
Answer the questions to document the teaching performed, patient response and plan.

Click Prognostics.
Click on one or more Prognostic Indicators in the Prognostic Graph tab.
a. Questions in green are prognostics.
Prognostics Factors may be added manually.
a. Click Edit.
b. Using the number pad click the plus sign ‘+’.
c. Use the arrow keys and number pad add the appropriate values.
d. Click Save.
Prognostic Factors may be viewed in many ways.
a. Select Prognostic Indicator.
b. Select Graph Type.
c. Select Date range.
Patient Education Handouts

a. These are FULLY CUSTOMIZABLE Microsoft Word documents
 edit them for your practice.
 add a Logo.
 add Contact Info.
 edit content.
 Select the forms (i.e., ONCOCHART radiation Therapy To the ….).
Open and Print as patient handouts.

Click ePrescribe.
Add patient Allergies.
a. Select Common Allergies from the dropdown or add Common Allergy Type.
b. Click Done with Allergies.
Add patient pharmacy.
a. Fill in the zip code and state, and then click Find.
b. Any pharmacy with (E) next to the name is an electronically prescribed pharmacy.
c. Click the pharmacy name to add to patient chart.
Add patient prescription.
a. Type all or part of the name of the medication and click Find.
b. Select desired medication from the list.
c. Fill out Sig Code information, quantity, refills etc…
d. Click Continue. Verify that your script is correct.
 You can add your prescription to your favorite list for quick access in
future.
 Click OK to finalize adding a prescription.
e. At this point, the prescription is ready for the physician to approve.
Select prescription you want to transmit. Type your signature password and click send.
(For schedule medications, they can’t be electronically sent but can be entered into the
system for documentation).
Add patient outside medication.
a. Click the Manage Meds hyperlink.
b. Type all or part of the name of the medication and click Find.
c. Select desired medication from the list.
d. Change provider to Select One.
e. Type in comments who prescribed the medication and click Continue.
f. For a detailed report of medication prescribed by outside physicians, click PBM Hx
hyperlink, obtain new data and select the medication you want to add to the outside
list.
g. Close ePrescribe tab and medications will populate your patient’s ONCOCHART®
records.
