Overall Purpose of the Job
The staff member in this position provides daily oversight, development and facilitation of the transitional and
independent living programs, including on-site support and supervision of residents, organization and upkeep of
physical facility, and educational and personal development of program participants. It is a live-in position. Core
responsibilities include resident and community education and training; transitional and independent resident case
management, advocacy, and community coordination; development and oversight of transitional and independent
living programs; intake, assessment and processing of transitional and independent living referrals; and on-call,
afterhours, and emergency response, including behavioral emergencies in the homes and emergency intake and
placement. There will be other duties as required identified by the supervising manager.
Major Functions and Responsibilities

Resident Education and Skills Development
30%
a. Provides counsel and skills training to residents, both 1:1 and in groups, to build life skills, future
plans, and ability to successfully transition into independence. This includes training on ability to
manage money, job skills, interpersonal boundaries, educational development, and all other
elements of independent living and self-care.
b. Utilizes live-in status and consistent presence in home to monitor and take advantage of
opportunities for naturalistic learning and skills development.
c. Creates opportunities for residents to expand independence and practice important life skills in a
controlled and safe environment.
d. Role model and leader for residents through actions, behavior, language, and dress.
e. Researches and gathers latest information and best techniques to enhance the quality and
effectiveness of resident training.
f. Plans a group and individual training agenda and schedule driven by goals on Plans of Care,
transitional learning plans, and needs of resident population.
g. Works individually in homes and community with MR/DD and special needs residents to build job
skills, independent living abilities, emotional resiliency, and interpersonal skills.

Household Supervision and Organization

a. Enforces rules and structure of the home through monitoring, supervision, and delegation of
appropriate chores and responsibilities to residents.
b. Maintains household schedule of events, activities, and appointments.
c. Responsible for insuring cleanliness, upkeep, and safety of the home and surrounding property,
including required documentation of monthly checks and drills.
d. Assists residents in scheduling, planning, and preparing their meals.

Resident Case Management and Advocacy

a. Monitors residents for continued stability, autonomy, health, safety, and progress toward
meeting goals and milestones.
b. Coordinates with residents, DSS social workers, LINKS Coordinator and the service team to
identify and address needs and goals identified on Plans of Care and transitional learning plans.
c. Plans, writes, and continuously updates resident Plans of Care, including formulation of goals,
services, and community supports.
d. Attends required meetings and court hearings and makes community contacts to keep up to date
and relevant information on all residents and help effectively plan for their needs.
e. Conducts regular assessments of resident life skills, functioning, and needs in order to determine
best course for services, programs, and treatments.
f. Identifies and leverages community resources and partnerships to find needed services and
supports that cannot be addressed by the Children’s Homes alone.

Programs and Services Development and Management

a. Researches and integrates new approaches and best practices in planning and development of
the Transitional Living Program and independent living program.
b. Develops support systems, documentation, and procedures to help organize, manage, and track
agency services and programs.
c. Coordinates with direct care staff and community stakeholders to integrate agency services,
community activities, and other enrichment opportunities into routine and schedule of homes.
d. Modifies, tailors, and adapts program services and delivery methods as required to address each
resident’s unique circumstances and meet special developmental needs.

Administrative Duties and Performance Improvement

a. Writes detailed daily notes of contacts and services provided to document progress toward
meeting resident goals and outcomes of services.
b. Assists with initial intake and processing of new resident referrals, including pre-screenings,
information collections, and completion of required documentation at the time of placement.
c. Monitors household budgets and expenses, staying within designated limits and submitting
regular documentation and financial reports in a timely fashion.
d. Assists with collections and tracking of data for the agency Performance Improvement Cycle.
e. Tracks outcomes related to skills training and development.
f. Assists with documentation and filing of necessary paperwork to maintain agency licensure and
national accreditation.

Assists in researching, tracking, and monitoring agency outcomes, including longitudinal
aftercare monitoring and study of residents following discharge from care.
Builds community awareness and expands possible referral sources outside Iredell County.
Submits reports as required by the Director of Residential Services and other agency leadership.

Resident Transportation

a. Provides transportation for residents to important meetings, activities, and appointments,
insuring the best, safest, and most economical use of agency vehicles.
b. Insures agency vehicles are maintained properly and kept clean.
Required Skills and Abilities






In depth understanding of learning styles and various approaches to skills training.
Extensive knowledge of at-risk youth and problems facing children in foster care systems, specifically
those aging out of care.
Extensive knowledge of MR/DD and special needs population.
Computer skills, including MS-Office Suite.
Strong written and spoken communication skills.
Ability to manage complex schedules and competing needs.
Education and Other Prerequisites




Bachelor’s degree in social work or other related human services field.
5 years experience in direct care coordination or supervising care for youth.
5 years experience working with a special needs or MR/DD population.
Possession of a valid state issued driver’s license.
Physical Requirements
While performing the responsibilities of the Transitional Program Coordinator position, the employee is required
to talk and hear. The employee is often required to sit and use their hands and fingers, to handle or feel. The
employee is sometimes required to stand, walk, reach with arms and hands, climb or balance, and to stoop, kneel,
crouch or crawl. Vision abilities required by this job include close vision. This position requires some light to
moderate lifting and possible exposure to communicable disease. We are prepared to consider any reasonable
