Dissemination of Psychoeducational Reports Upon the completion of an initial evaluation or reevaluation, the school psychologist will make copies of the psychoeducational report. 1) School Psychologist a) The school psychologist shall retain the original copy of the psychoeducational report for his/her use during the initial eligibility/reevaluation meeting. b) Upon completion of the initial eligibility/reevaluation meeting, the school psychologist shall return this copy of the report to their special education secretary at the district office. 2) Parent a) The school psychologist shall provide a copy of the psychoeducational report for the parents. b) If parents request to see the report prior to the meeting, the school psychologist will instruct them that they may pick up the report from the school psychologist at one of the school psychologist’s service schools, on a day on which the school psychologist will be at the school. If the parent requests that the school psychologist fax the report to him/her, the school psychologist may fax the report. Psychologists may not send psychoeducational reports to the parent via student or via email. c) If the parent does not obtain the report prior to the meeting, the school psychologist will bring the parent’s copy of the psychoeducational report to the initial eligibility/ reevaluation meeting. If the parent does not attend the meeting, the school psychologist will ensure that the parent copy of this report is shredded. The parent may then request a copy of the psychoeducational report from the district office. d) If, prior to or at the eligibility meeting, parents request more than one copy of the psychoeducational report, the school psychologist will make the requested number of copies and provide them to the parent. 3) Other IEP Team Members a) Upon request, the school psychologist shall provide a copy of the psychoeducational report to other IEP Team Members (such as the special education teacher, regular education teacher, speech/language pathologist, and administrator). Note: Reports may be disseminated for professional purposes only, solely to those personnel who have a legitimate educational interest in the records, in order to help with program planning. b) To receive a copy of the report, the IEP Team Member must complete the Access to Psychoeducational Reports form, listing the student’s name, date of birth, the staff member’s name, and the reason for access. The IEP Team Member must indicate the date on which they sign out the report, as well as the date on which they return the report to the school psychologist. Psychoeducational reports may not be emailed to IEP Team Members; however, the school psychologist may email academic information (including, but not limited to, DIBELS, MAP, PASS, and individually administered achievement test scores, as well as a description of academic strengths and weaknesses) to IEP Team Members. c) IEP Team Members may not make copies of the psychoeducational report. d) The school psychologist will ensure that any copies of the psychoeducational report disseminated to IEP Team Members are shredded upon their return to the school psychologist. Student Access Upon completion of an initial evaluation or reevaluation, the school psychologist may discuss results of the evaluation with the student, at a developmentally appropriate level. If a student is under the age of majority (age 18) and requests a copy of the psychoeducational evaluation report, then the school psychologist shall contact the parent to inform him/her of the student’s request. At the age of majority, unless a student has legally given his/her rights to the parent, the school psychologist shall provide a copy of the psychoeducational report to the student without parental notification. Requests for Psychoeducational Reports After Initial Eligibility/Reevaluation Meeting Any requests for psychoeducational reports after initial eligibility/reevaluation meetings must be made through the school’s special education secretary at the district office.
