PLEASANTS COUNTY SCHOOLS ‘CHEAT SHEET’ FOR INITIAL EVALUATIONS After student has been referred for a multi-disciplinary evaluation: School psychologist or central office obtains permission for evaluation. As soon as possible after obtaining permission – School psychologist or central office will complete MDET Checklist and provide to school personnel for completion. By date indicated on MDET Checklist (no more than 35 days after receiving MDET Checklist) – School completes required evaluations (including achievement testing, observations, teacher reports, etc.) as requested on checklist and submits to Central Office, Attn. Kim Kehrer. At least 45 days prior to eligibility due date – Completed school packet submitted to school psychologist. School psychologist completes testing. Within 20 days of beginning psychological evaluation – Psychologist must complete evaluations and write report. One week prior to eligibility meeting – Psychologist conducts a “pre-meeting” with the family to review psychological testing. Within 80 days of permission to evaluate and no more than 35 days after beginning psychological - Eligibility meeting must be held. Tricia Miller schedules eligibility meetings for initial evaluations. 15 minutes prior to meeting – Review Procedural Safeguards with parent (if appropriate.) This can be done by principal, special education teacher, school psychologist, speech therapist, PERC Coordinator, or other knowledgeable individual. PCS/kk Initial Eligibility Checklist 08/15
