Child Abuse/Neglect or Dependency Cases
If abuse/neglect or dependency (the lack of ability to care for a child due to extenuating
circumstances) is suspected you should report the case to the Children Services agency in the
county of residence.
The mandatory reports are when you suspect sexual abuse, positive toxicology screen for a child,
signs of physical abuse – fractures, bruising, etc, severe malnourishment
The intake specialist will want the following information:
Child’s Name
Child’s Address
Child’s Date of Birth
Parent/Guardian contact information
Description of the situation, to the best of your knowledge.
Identify that the child is in an emergency room setting and what the intended
medical
disposition is.
Identify if there is concern for discharging the child to the home.
If available, the intake specialist would also like:
Names and ages of other siblings in the home
Whether there are firearms or vicious dogs in the home
Whether any other social service agency is involved with the child or family
The intake specialist will take the information and determine if there will be any intervention. If
the situation is serious and warrants immediate attention they will tell you that and instruct you as
to what to do with the child/children.

Domestic Violence Cases
These cases are not a part of the mandatory reporting cases unless there is serious physical harm.
Serious physical harm is defined as:
Any mental illness or condition of such gravity as would normally require
hospitalization or prolonged psychiatric treatment.
Any physical harm that carries a substantial risk of death
Any physical harm that involves some incapacity
Any physical harm that involves some temporary, permanent or serious
disfigurement
Any physical harm that involves acute pain of such duration as to result in
substantial suffering or that involves any degree of prolonged or
intractable pain.
The patient must give consent for a health care professional to report Domestic Violence if one of
the above is not met. The verbal consent should be documented in the medical record.
The Domestic Violence report can be made to the presiding local law enforcement.

Most of these cases require just the distribution of resources. Providing the patient with the
above appropriate law enforcement agency for the patient to follow up with if they wish to report
at a later date or the other resources listed in the attachment to the Victims of Abuse policy.
Elder Abuse
For cases where it is suspected that a person age 65 or older has been abused, neglected or being
taken advantage of due to age impairment or cognition, referrals are made to the Adult Protective
Services Agency.
The intake specialist will want to know:
Patient’s name
Patient’s address
A description of the situation or injuries noted
Disposition of the patient – being admitted or discharged to home.
The intake specialist will take the information and it will then be processed with a supervisor and
they will decide what interventions need to occur. The agency will only take the referral if they
are being released to home. If they are being admitted, contact the Quality Coordinator and they
will make the referral if appropriate at time of discharge.
