26th February 2015 Web: www.merseycare.nhs.uk Dear GP Colleague To improve the communication between North Liverpool CMHT and our GP colleagues, we are continuing to implement a programme of visits to all Practices in our catchment area. The visits will continue to be led by the Primary Care Mental Health Liaison practitioner for North Liverpool, Andrea Balfour. The previous evaluation in March / April 2014 showed that this collaboration is valued, and is beneficial to primary care services / patient outcomes. The Practitioners will continue to work closely with all GP Practices to: Improve collaborative working between primary care/secondary mental health services (Including Adult and Older Adults and Early Intervention terms) Improve health and communication outcomes. Engage with GPs across specific localities regarding physical health needs of those with mental illness and other long-term conditions. We hope that practices will continue to welcome the Practitioners to practice meetings to discuss the general health and well-being of patients on the practice’s SMI register and any queries that practice staff may have. To assist in this process, we have updated the previous quick reference list to include information on Learning Disabilities, which also includes: 1. The process of referral to the Acute Care Team and Older Peoples Community teams. 2. Contact details for key staff at North Liverpool CMHT. The Learning Disabilities Service provides a specialist health service to men and women, over the age of 18 years old with learning disabilities. The team can provide a comprehensive assessment to identify needs and from this an intervention plan in response to these needs. Please circulate the contents to all doctors and relevant staff working in your Practice. Andrea will also be updating the Information Packs which contain details about the services and the process of referrals to those teams and will be delivering them to the individual practices.
