Lessons for Life United States Professional Tennis Association 2016 Awards Program 2016 USPTA Lessons for LifeTM Award USPTA’s Lessons for Life Award recognizes those USPTA-certified members who have done the most to raise money for charities through tennis events. While the award spotlights individuals who have raised large amounts of money, it also honors those hosting the most creative, original or unique tennis event to raise money for charities. An individual hosting an event raising the most money will not necessarily receive this award. Please keep in mind that while this is not limited to tennisspecific events, an individual hosting a massive event including tennis, golf, etc., at a club will need to provide a breakdown of what portion of the proceeds the tennis event brought in. To qualify for this award, the event(s) must be registered with USPTA. General rules 1. Divisions are asked to nominate one person each year for this award. Nominations also are accepted from USPTA members or nonmembers. Nominees must be USPTA-certified members (Master Pro, Elite Pro or Professional Level) in good standing. 2. Every question on the application must be answered. Please type. Additional, supporting information may be included as backup, including program information, news clippings, etc. 3. The awards period is from January 1, 2015 – December 31, 2015. 4. ALL applications can now be submitted as PDFs and submitted via email. The Awards Committee encourages applicants to submit their applications and all supporting materials via email, Dropbox (www.dropbox.com), Google Drive, or a similar electronic service. 5. The deadline for the USPTA World Headquarters to receive all 2016 award applications is February 28, 2016. Having an application postmarked February 28 is not acceptable and those applications will not be considered by the Awards Committee. Applications submitted via email must be emailed by February 28, 2016 at 11:59 p.m. Central time. The entire application and ALL supporting materials must be received on or before the deadline date to be considered by the Awards Committee. Some USPTA awards may or may not be given each year, at the committee’s discretion. Recipients will be announced/recognized during the 2016 USPTA World Conference in Indian Wells, CA. Award recipients are encouraged to attend the conference to receive their awards, however, not all awards will be presented at the conference. 8. A certified member may nominate himself or herself, or another certified member, as long as the application form provided by USPTA is used. To receive credit for the nomination, a division must include the completed application in its electronic submission as well.
