VCOM 3rd YEAR PEDIATRICS ROTATION PATIENT LOG
Site Location/Preceptor_____________________
Student Name ________________________
Rotation Dates ___________________________
WELCOME to your Pediatric Clerkship!!! During this time with your preceptors you will see a wide
variety of well and sick children. It is very important you keep a log of your encounters by using
this log during your pediatric rotation. This is certainly not a comprehensive list but just some of
the common/high yield pathologies that you may experience. Instructions for completing your
required patient log are below:
-Because we know each experience with your preceptor may be different than your classmates, we
have included the number of the CLIPP case(s) that correspond to the patient diagnosis we want
you to be familiar with, this allows you to complete a CLIPP case as part of your patient log if
needed.
- You are required to complete a minimum of 11 CLIPP cases to pass the rotation. But it is highly
recommended that you complete all the CLIPP cases for your own learning and for optimal scores on
your end of rotation exam. Although you must complete all the CLIPP cases during your pediatric
rotation, you will NOT be able to use the CLIPP cases as your complete experience under any
category. For example, if you need to see 3 skin cases all 3 of them cannot be from a CLIPP case.
***The only exceptions to this rule are the cases under CNS (As these are not as commonly seen in
the office, you may use the CLIPP cases for your entire CNS requirement if you do not see an
actual patient with a CNS diagnosis in the office).
-Sometimes it may be difficult to see a variety of cases in the office setting for each category, so
if you must complete a CLIPP case as a requirement, that CLIPP case MUST be a diagnosis you did
NOT see on your rotation.
-If you see an interesting patient with a diagnosis that is not listed as part of your
requirement, please include it and have your preceptor sign off on your log. We will take each
additional case into consideration as a substitution to another diagnosis that you were not able
to see under a certain category.
-In pediatrics, comprehensive well child care is a primary goal for any pediatrician or family
medicine physician. For this reason, we want to make sure you are paying careful attention to
these visits with your preceptors. There will be no CLIPP case substitution for these encounters on
the patient log and you should see 4 DIFFERENT ages in order to fulfill your requirement.
Your patient log should be submitted on-line at the end of your rotation and will be part of your
evaluation for the clerkship. You are responsible for completing your log. The CLIPP cases are
designed as a learning tool and should be utilized in preparation for your test at the end of the
rotation.
HAVE A GOOD TIME ON YOUR ROTATION. MAKE YOURSELF ALWAYS AVAILABLE TO
LEARN. PEDIATRICS IS FUN AND WITH THE RIGHT APPROACH AND ATTITUDE, YOUR
ROTATION IS AN EXCELLENT OPPORTUNITY TO LEARN AND GROW AS A MEDICAL
STUDENT!!!! SANITIZE YOUR HANDS FREQUENTLY; DON’T TOUCH YOUR FACE AND STAY
WELL!
