Fatima Chaudhry Assessment Project Problem: 1. To help reduce maternal and neonatal mortality rates through proper prevention, vaccination, exercise and dietary restrictions. Refining the Problem: 2. This evaluation will be used to assess health, nutrition and vaccination habits of pregnant mothers or mothers with young babies. This evaluation is going to promote optimal child growth and development, enhance the body’s ability to fight infections. 3. This evaluation will be used to help mothers and children to achieve and maintain optimal body weight, make their medications more effective, prolongs good health, and improve quality of life. 4. The components I have chosen to evaluate are dietary habits, vaccination schedule, and exercise. By assessing this one will be able to predict how well the new born or new born to be will fare. All of these components have a direct correlation to the health of the baby. Instrumentation and Methodology: 5. Each client will be evaluated on a scale of 1-5 in all the components listed above. A score of 1 will mean that the child is in high risk of having an unhealthy start to life and a score of 5 will mean prime position for new born or new born to be leading a healthy lifestyle.. A score of 2 would mean that they are below average in that category. A score of 3 would mean that the child is average in that category. A score of 4 would mean that the child will do above average in that category. A score of 5 would mean that the child did excellent in that category. No one should be below average. 6. The test administrator would have to know how to asses heartbeat, blood pressure, bodies reactions to certain exercises that deal with strength and fitness. Also the administrator would need to know how to use a weighing scale, how to use calipers to properly find body fat percentages. The administrator would also need to know how to operate safely basic exercise equipment such as treadmills and weight machines. The administrator should also know how to read and properly judge vaccination, and medication histories. The administrator should be able to correlate all these components along with their impact on physical health. Analysis: 7. Based on the ratings for all three components which will be evaluated we will average out the score and based on the score we will take equivalent actions. If the baby is at high risk of unhealthy lifestyle we will take appropriate preventative measures. If the baby is at low risk then we will tell the mother to stay on track and consistently come back to our assessment. This will be a formative assessment.
