Annual Enrollment Contract: 2015-2016
I/We, the undersigned Guarantors, being all of the natural parents and/or legal guardians
of the below listed student(s), in consideration of the acceptance of this annual enrollment
contract and enrollment in Saint Mary’s Catholic School, represent, request, understand and
agree as follows:
1. Requirement for Contract. Guarantors understand that all student enrollment is done by
the annual enrollment contract pursuant to Diocesan policy. Guarantors understand that
the School relies upon contracts to plan and budget for the upcoming year, and, that any
breach of contract results in an economic hardship for the School.
2. Proposed Student(s). Guarantors request enrollment of the following student(s) for the
upcoming school year.

3. Contract Due Date. Guarantors understand and agree that this contract is due in the
School’s office, fully executed with deposit payment, two weeks after receipt.
4. Finality of Contract. Guarantors understand and agree that this contract becomes binding
once it is accepted by the School. Acceptance occurs once this contract is endorsed by the
School Principal or other authorized School official.
5. Deposit. Guarantors understand and agree that a non-refundable admission fee of $350.00
per child is due in the School office upon delivery of this contract and is a condition to
acceptance of this contract.
6. PTO Membership. Guarantors understand and agree to membership in the Saint Mary’s
Parent-Teacher Organization (PTO) and agree to abide by rules and programs promulgated
by the PTO Board.
7. Tuition. Guarantors understand and agree to pay the total cost of tuition of $. _______
(The appropriate tuition amount may be obtained from the schedule at the end of this
contract.)
Guarantors understand and agree that the School offers only two payment plans. Guarantors
have indicated their preferred method of payment by checking either “A” or “B” below:
A. _______ Payment in full by June 30, 2015. In doing so, Guarantors understand that
they will receive a one percent discount of the total cost of tuition as set
forth in the attached tuition schedule.
B. _______ Monthly payment by FACTS Tuition Plan. Guarantors agree to execute all
FACTS contract documents and deliver the same upon delivery of this
contract as a condition to acceptance of this contract.
8. Default. Guarantors understand and agree that they will be in default of this contract under
any of the following events:

Any payment due under this contract is thirty or more days late.
Any check tendered for payment is returned unpaid.
Any FACTS Tuition Plan payment due is unpaid by due date.
Any student indicated in paragraph two is dismissed.
Any student indicated in paragraph two is withdrawn.
In the event of default, all tuition, fees and costs incurred become immediately due and payable.
9. Dismissal. Guarantors understand and agree that it is important that Guarantors and
students accept and fully cooperate with the School in regard to all matters of rules,
regulations, and discipline. Guarantors understand and agree that the School reserves the
right to dismiss or suspend from the School during the term of this contract any student
whose progress or conduct is unsatisfactory and not in the best interest of the student
and/or the School, as determined in the sole discretion of the School.
10. Rights of Non-custodial Parents. Guarantors understand and agree that the School will
recognize access to Students and their records by non-custodial parents unless a court order
denying such access is timely provided to the School.
11. Complete Terms and Modifications. Guarantors understand and agree that the contract
contains all of the terms agreed to between them and the School, and, that the contract can
only be modified, altered or changed by a written contract modification signed by all of the
parties.
12. Understanding and Compliance. Guarantors represent that they have read and understood
all of the terms in the contract, and have complied by providing all of the information
required in this contract.
