Unofficial Transcript Evaluation for OSU Pharm.D. Prerequisites Worksheet We encourage prospective students to submit a request for a transcript evaluation to determine how your coursework applies to the Pharm.D. prerequisites. In order to receive an unofficial evaluation of your transcript(s), please fill out the following worksheet and submit transcripts from all institutions you attended. You can submit copies of this worksheet and your transcripts via e-mail to pharmacy@oregonstate.edu, fax 541-737-3999 attn: Transcript Evaluation or mail (Transcript Evaluation; 203 Pharmacy Building; Corvallis, OR 97331). Unofficial transcript copies are sufficient for the informal evaluation. Please allow four to six weeks to receive the results of your evaluation. Results will be sent by e-mail to the address you provided. Transcripts will not be returned. Instructions for completing the Pre-Requisite Worksheet (for Transcript Evaluation): Please see the sample table below that includes examples of how to properly fill out the following worksheet. If you withdrew from or repeated a prerequisite course, you must list each enrollment on this form (see example below). List the number of credit-hours for each course in quarter credits. To convert semester credits to quarter credits, multiply the semester credits by 1.5 (e.g. 3 semester credits equals 4.5 quarter credits). Leave the "Grade(s)" column blank for in-progress and planned coursework. List AP exam score in the "Grade(s)" column if you wish to use AP credit to satisfy that requirement (see example below). Use a separate line for each course. If you have taken one course that applies to multiple prerequisites (such as a combined anatomy and physiology course), list the course in both sections of the worksheet. Enter grades exactly as they are listed on your transcript. Do not convert numerical grades to letter grades.
