(c) I declare that the animals described above have/have not previously been vaccinated against Botulism, whilst on
the property at least once in their lifetime.
(d) I further declare :  the property has remained free from clinical or visible evidence of Leptospirosis and Leptospirosis has not been
officially reported during the 3 years prior to shipment.

Encephalomyelitis, Trichonomiasis, Ephemoral Fever, Q Fever & Clostridial diseases.

The original is then to be attached to Waybills and handed to transport driver for carriage to registered depot. It
is a requirement of the Australian Livestock Export Standards Version 2.3 2011 (S3.12) “when receiving and identifying
livestock, the Depot Operator must obtain a copy of the Vendor Declaration/s regarding the property of source and health
status of the livestock before accepting the livestock for the purpose of preparation for export”.
PLEASE NOTE: You are advised that this document will be presented to an DAFF Certifying Officer for the purposes of
the Export Control Act No 47 of 1982 as amended. Giving false and misleading information is an offence, with penalties
of up to 5 years imprisonment

For undertaking pregnancy testing in the Northern Territory a registered veterinarian or a competent
pregnancy tester, accredited by the NT Governemnt are able to conduct pregnancy tests. For pregnancy
testing in other states cattle and buffalo must only be tested by a registered veterinarian. If speying is the
preferred method, then the declaration is to be completede by the owner / manager.
