Readmission Reduction/ToC Improvement Strategy Guide
Key Elements
Element One: Targeted Approach – Hospitals that have been working on reducing readmissions for some time without
seeing results. Consultation with team or hospital site visit with the Transition of Care / Readmissions WHA
Improvement Advisor to review work done to-date and to consider alternate approaches to achieve results. This is a link

Element Two: Transitions of Care / Readmissions Reduction Webinar Series evidence base and best practice sharing
webinars addressing specific topics will be held in even months. Coaching calls will be held in odd months.
Element Three: Essential Safety Topics – Webinars focused on key cross cutting safety topics that can impact
readmissions reduction. For ToC/READ the topics include: White Board Use, Bedside Shift Report, Multi-disciplinary
Rounds.
Element Four: Implementing a Protocol - A change management approach designed to close gaps in best practices of
care. Consider this approach when there is a need to ‘hard-wire’ particular best practices for prevention of
readmissions.
