Please keep it for reference. We are a group of six doctors serving the area shown on the map at the back of the booklet. We aim to provide a quality family doctor service for residents of Shaw. They have to keep the patients and the doctors happy! You may consult with the doctor of your choice during the above surgery times. Emergencies will be seen the same day. If you cannot attend for your appointment please let us know as this appointment can then be given to someone else. Emergency Surgery runs every morning Monday to Friday. This is for patients with an urgent problem who cannot wait for a routine appointment. Patients in this surgery must see any available doctor and must be prepared for a possible wait. HOME VISITS – Please telephone 01706 888222 Where possible, we ask that a request for a home visit be made before 10.30am. Be prepared to give brief details of the problem to the receptionist to enable the doctor to assess the urgency of your call. Please remember, visits are for people too ill to attend the surgery. It is not possible to request a specific doctor to visit. The doctor may telephone you before the home visit to discuss your condition. NHS 111 - Out of Hours – evening and weekends If you require urgent medical assistance that cannot wait until the Surgery re-opens, please telephone 1-1-1. Calls to the NHS 111 service are free from both landlines and mobiles. If you have a life threatening emergency please dial 999. The Surgery will be open again from 8am the next working day REPEAT PRESCRIPTIONS If your doctor agrees, you may obtain repeat prescriptions. Please indicate which items you require on the tear off slip and place it in the box provided. 48 hours notice is required for a repeat prescription. Some local chemists will collect prescriptions on your behalf. You can also order your repeat prescription on line at www.TheOakGablesPartnership.co.uk or register for EMIS patient access. SERVICES PROVIDED BY THE PRACTICE Family planning and pre conception advice Contraceptive services including the fitting of intrauterine devices Cervical screening and Maternity services Child health surveillance including childhood immunisation Diabetes, Asthma, Coronary heart disease and Stroke monitoring Holiday planners and immunisation for foreign travel Adult immunisation including flu and pneumococcal vaccination Minor surgery and joint injection Menopause and HRT monitoring Over 75’s monitoring, new patient and well person checks Smoking cessation advice and blood pressure monitoring TRAINING PRACTICE We are recognised as a training practice. Doctors from the hospital may spend up to four to six months in the practice. These are qualified doctors and this is part of their ongoing training. We are also recognised by Manchester University as a teaching practice for medical students. ACCESS AND FACILITIES FOR THE DISABLED Designated disabled parking is available at the rear and side of the Health Centre. There is a ramp access to the main entrance. A disabled toilet is available. SUGGESTIONS/COMPLAINTS/COMPLIMENTS We aim to provide a personal, friendly and efficient service. If you have any comments or suggestions regarding the service we provide we would be happy to hear from you either in person or in writing. Sometimes a patient may have a cause for complaint. Please contact our Practice Manager who will outline our complaints procedure and endeavour to resolve the problem. NEW PATIENTS All new patients are required to complete registration formalities and are offered an appointment with a nurse to enable the surgery to make a full assessment of your medical needs. On registering with the practice you may wish to express a preference of which doctor you would like to be registered with. Please let the receptionist know, and we will make the necessary arrangements. We ask for information about you so that you can receive the best possible care and treatment. We keep this information together with details of your care to ensure that your doctor, nurse or health care professional has accurate and up to date information. There are such times when we have to pass on information to other people such as hospitals, social services, primary care trust etc. This is always done confidentially. PATIENT’S RESPONSIBILITIES All patients are asked to be co-operative and act in a civil and courteous manner. We ask that all patients: Inform the practice if they have changed address or telephone number Only make appointments you know you can keep Read the practice leaflet that provides details of our services Be patient if the surgeries are running late Please remember that one appointment is for one person The practice adopts a procedure of non tolerance regarding abusive, threatening or aggressive behaviour directed towards the doctors, practice staff or other health care professionals who work with us. Any incident of this nature may result in the patient being removed from the practice list. CHANGE OF ADDRESS OR TELEPHONE NUMBER If you move home please advise the receptionist of the new address and telephone number as soon as possible to help keep our records up to date. Please remember, if you have call barring on your telephone, or your number is X-Directory, the practice will be unable to contact you at any time. If you move outside the area it will be necessary for you to register with a practice nearer to home. ADDITIONAL SERVICES AND INFORMATION Chaperone policy In certain circumstances during an examination of an intimate nature, the doctor may request a chaperone to be present. This is to protect both the doctor and the patient. A patient may of course also request a chaperone to be present. Private medicals and reports The NHS does not cover certain medicals, reports and forms. These will be charged at the BMA recommended rate. Please ask at reception for details. No smoking policy There is a no smoking policy within all areas of the Health Centre. Mobile telephones Please switch off all mobile telephones when you enter the building. If a mobile telephone rings during a consultation you may be asked to leave the doctor’s surgery. NHS Walk in Centre The nearest walk in centre is the ICC Centre in Oldham. This is situated next to the Civic Centre in Oldham. The centre is open from 8.00am to 8.00pm. Data Protection Patients are advised that information about themselves will be used strictly in accordance with the purpose or purposes for which it has been collected. Patients are advised that the practice is committed to the overall principle of the Data Protection Act, to ensure confidentiality, availability and integrity of patient based data. The practice operates on the principle that confidential information will only be disclosed to other parts of the NHS where it is felt to be in the best interest of the patient. PATIENTS CHARTER Patient’s Rights Patients at this practice are entitled to: Be treated in a courteous, civil and co-operative manner. Be given a clear explanation of, and advice on their medical condition and any proposed treatments. A referral for specialist medical opinion or other second opinion when appropriate. Absolute confidentiality of medical records maintained by the practice subject to a provision for disclosure imposed or allowed by law. Access their own medical records, in accordance with the Data Protection Act. Please enquire at reception for details. Have any formal/informal complaint against the practice investigated and dealt with fairly in accordance with our complaints procedure. Request to be seen by a specific doctor or health care professional. It may not always be possibly to comply with a request where the specific doctor is unavailable. Patients aged 16-74 may have a health check every three years by request. Patients over the age of 75 may have an annual health check at home if necessary.
