Registered Pharmacist
Department:
Pharmacy
Job Summary:
Interprets physician medication orders and prescriptions.
Dispenses medication prescribed and other pharmaceuticals and
acts as an integral part of the medication management process.
Acts as a drug information resource to patients, medical staff,
nursing staff, and ancillary department staff. Pharmacists shall
comply with all applicable hospital policies, procedures and
standards for the provision of medication in the hospital’s
medication management process. Pharmacists shall work under
the supervision of the pharmacy director.
Job Qualifications:
A.
Education:
B.S. in Pharmacy or Doctorate of Pharmacy from an
accredited College of Pharmacy.
B.
Personal Job-Related Skills: Ability to assist the pharmacist in provision
of medication for the medication management process. Effective oral and
written communication skills
C.
Licensure, Registry or Certification: Current Registered licensure by the
Texas State Board of Pharmacy.

Experience:

Prior work experience: One year experience as a Registered
Pharmacist. Hospital experience preferred.
2.
Technical Training:
Current CPR certification.
IV Certification as required by TSBP
Physical and Mental Requirements: Duties of the position require
extended periods of standing and walking. Bending, stooping, twisting,
reaching and lifting up to 40 pounds.
Essential Duties and Responsibilities:
A.
Comply with all applicable hospital policies, procedures and standards.
B.
In the absence of the director of pharmacy, may be required to perform
essential duties of the director of pharmacy.
C.
Interprets and fills physician’s orders.
D.
Maintains patient medication records.
E.
Monitors drug therapy for contraindications, incompatibilities, allergies
and dosing errors.
F.
Issues controlled substances to patient care areas and maintains records as
required by law and pharmacy policies.
G.
Provides drug information to the hospital’s professional staff and patients
as necessary.
H.
Participates in stock control programs (ordering, inventorying, and
monitoring usage) as required by the director of pharmacy.
I.
Assists in monthly inspections of drug storage as required by the director
of pharmacy.
J.
Participates in pharmacy orientation programs, pharmacy staff meetings,
and in other programs, committees, meetings and functions as required by
the hospital or pharmacy.
K.
Prepares IV admixtures as required.
L.
Participates in Quality Assurance-Performance Improvement activities as
required.
M.
Participates in monitoring for effects on medication on patients such as
kinetic dosing.
N.
Demonstrates the knowledge and skills necessary to provide care to
patients of all age groups.
Other Significant Requirements:
A.
Completes position responsibilities by following established guidelines
and protocols within the appropriate time frame.

Maintains confidentiality while working with confidential matters on a
daily basis.

Demonstrates good skills in all forms of communication. Works
effectively with colleagues in accomplishing goals and objectives of the
department and organization. Provides services direct to patients while
maintaining a positive patient relationship and a supportive atmosphere.
D.
Maintains ACPE education requirements for position and completes
educational requirements as required by the hospital.
E.
Follows aseptic technique and established procedures for performing
admixtures and using precautions when involved in handling potentially
infectious materials.
F.
Adheres to established hospital and department guidelines and protocols.
Supports hospital mission, vision, and values.
G.
Performs job specific functions at an acceptable level.
H.
Provides excellent customer service to all customers of the pharmacy.
I.
Responds to emergency situations as needed.
