UCA College of Fine Arts and Communication Outstanding Student Award 2015 Guidelines To nominate students for the CFAC Outstanding College Student Award, faculty should send names of nominees to the dean’s office by noon, Wednesday, January 14, 2015 by e-mail to jdmiller@uca.edu or hand-delivered to Burdick Hall 210. Minimum qualifications for this award are as follows (please confirm before nominating): 1. The student nominee must have a declared major in the College of Fine Arts and Communication. 2. Senior Status. In 2015, the student nominee must have 90 or more earned hours of course work, 30 hours of which must have been earned at the University of Central Arkansas. 3. GPA of 3.25 or higher. The student nominee must have an overall grade point average of 3.25 or above through the fall semester of the current academic year. 4. Scholarly and/or Creative Achievements. The student nominee must have a strong record of scholarly and/or creative achievements. 5. Leadership or Service. The student nominee must have demonstrated exceptional leadership ability or rendered active service to the College of Fine Arts and Communication or organizations and activities sponsored by departments within the College. Nomination Process: CFAC faculty should send the student’s name and student ID number to the dean’s office by noon, Wednesday, January 14, 2015. The dean’s office will notify the student of their nomination and supply each with an application. It is of vital importance that the nominating faculty member obtains a current e-mail, mailing address, and phone number for the nominated student and provides that information to the dean’s office, failure to do so may result in the student not receiving a nomination notification letter from the dean. Application Process: Nominated students must complete an application consisting of the following documents: 1. Completed application form (found at uca.edu/cfac). 2. At least one letter of recommendation sent directly to the Dean of the College of Fine Arts and Communication (Burdick Hall 210) from a faculty member in the applicant’s major department. It is also advisable that the applicant ask other faculty members or chairs to submit letters of recommendation (4 maximum). Letters must be received by the CFAC Dean’s Office by February 11, 2015. Self-nominated students should follow the same application process. Nominated and self-nominated students should turn in their completed applications to the CFAC Dean’s Office (Burdick 210) by Wednesday, February 11, 2015.
