PSReport 0602 December 30, 2014 Jorge Frieze, MD Department of Radiation Oncology Brookfield Medical Center 30 University Place Fort Worth, TX 76104 RE: Lawrence Johnson, Jr. Dear Dr. Frieze: This is a letter of introduction for the above-named patient. A 14-month-old male with a unilateral sporadic retinoblastoma of the left eye, who I am referring to you for consideration of external beam radiotherapy. According to the patient’s mother, she first noted a white reflex in the left eye several months ago. Initially, he was treated by Linda Sams, MD, his pediatrician, for a conjunctivitis. When he returned for followup, he was noted to have a white reflex in the left eye. He was then referred to Wills Eye Hospital in Philadelphia, and was seen on December 22 in consultation. Mother also noted that her son had been more clumsy – walking into walls and falling more often. When watching T.V. he must be close up and tilts his head towards the right. At Wills Eye Hospital he was seen by Dr. R. Sowers. He was found to have a unilateral sporadic retinoblastoma in the left eye, abutting the optic disc and measuring 5.9 mm. Some calcified and some viable seeds were noted. Examination of the right eye was normal. The therapeutic options were considered to be external beam radiotherapy verses enucleation. Dr. Sowers suggested that the patient receive external beam radiotherapy and his parents concurred. Thank you for agreeing to participate in the referred patients’ care. If I can provide any addition information, please feel free to contact me. Please keep me informed of his progress. At the completion of radiotherapy, I will provide the oncologic followup, and Dr. Sowers will provide the ophthalmologic followup. Sincerely yours, Albert J.
