NORMA SHARPE AWARD: In 500 words or less, identify and explain how the candidate has made historical and outstanding contributions to the music therapy profession. Please submit at least 3 additional pieces of supporting documentation as outlined below. RESEARCH/PUBLICATIONS AWARD: List the date, type and title of each research paper or written publication. Please submit each outline, abstract or summation of publication with nominations form PROFESSIONAL DEVELOPMENT AWARD: In 500 words or less, identify and list the presentations given by candidate. If articles or tapes of media presentations or proof of presentations (i.e., conference programs, flyers, presentation evaluations, etc.) are available, please submit with nominations form. At least two pieces of supporting documentation must be submitted with this nomination. PEER RECOGNITION AWARD: In 500 words or less, identify and explain how this candidate has used their innovation to advance and inspire the music therapy profession in Canada. Please submit at least one additional letter of reference with this nomination. STUDENT AWARD: In 250 words or less, identify the suitability of this candidate for this award along with two letters of reference. Other supporting documentation may be submitted (optional). AGENCY/INDIVIDUAL CITATION: in 500 words or less, identify and explain how the candidate has made special contributions to the field of Music Therapy. Please submit at least one additional letter of reference or another significant piece of supporting documentation with this nomination. The Awards Committee will be comprised of one executive officer, one past president and the ethics chair. Awards are based on merit and suitability and are not necessarily granted each year. The Awards Committee is not eligible for any of the awards listed. Recipients of these awards will be notified by April 15 and invited to the CAMT annual award ceremony and banquet, as a guest of the CAMT. Norma Sharpe Award The CAMT recognizes members that have rendered many years of outstanding service to our national association and the field of music therapy. This award may be made annually to not more than one member of the CAMT. Qualifications: 1. MTA membership in good standing with the CAMT; and 2. has demonstrated significant and historical contributions to the field of Music Therapy, i.e.: service on committees and boards which reflect advancement in our field, outstanding client care, advocacy work in the field of Music Therapy, development of workshops/presentations; and 3. demonstrates commitment to ongoing involvement in upgrading personal and professional knowledge related to Music Therapy. 4. At least three additional pieces of supporting documentation must be submitted with the nomination. This could include (but is not limited to) the nominee’s CV/resume, letters of support from colleagues, former students, etc.; copies of evaluations and/or positive feedback and/or awards received, list of presentations/publications, etc. The recipient of this award will receive a life time membership with the CAMT. Research or Publishing Award The CAMT recognizes individuals who have promoted our field through research or published writings. This award may be made annually to not more than one member of the CAMT. Qualifications: 1. MTA membership in good standing with the CAMT; and 2. research which has helped advance and/or promote the music therapy profession; or 3. authored/co-authored any book, papers or chapter of music therapy; or 4. published writings for the profession that has affected philosophy or practice. Professional Development Award The CAMT recognizes members who have enriched our field through their extensive presentations either within or outside of the music therapy community. This award may be made annually to not more than one member of the CAMT. Qualifications: 1. Current MTA membership in good standing with the CAMT; and 2. has made presentations on music therapy in person, or through the media; and, or 3. has been featured in the written media on topics pertaining to music therapy. 4. At least two additional pieces of supporting documentation must be submitted with this nomination. Peer Recognition Award The CAMT recognizes and supports excellence in the field of Music Therapy. This award may be made annually to not more than one member of the CAMT. Qualifications: 1. Current MTA membership in good standing with the CAMT; and 2. has taken a lead role in music therapy development (i.e., clinical work, private practice, education, community enhancement) 3. has inspired others in the field of music therapy 4. At least one additional letter of reference must be submitted with this nomination. The recipient of this award will receive a certificate of recognition and will be profiled in the CAMT newsletter. Student Award The CAMT recognizes students who have demonstrated strong academic ability, and demonstrate promise to contribute to the field of Music Therapy. This award may be made annually to not more than two students or interns. Qualifications: 1. Student membership with the CAMT; and 2. current enrollment in a CAMT approved undergraduate or graduate training program, and/ or internship in CAMT approved internship site; and 3. has demonstrated high academic achievement as well as extensive volunteer or work experience in music therapy and/or non-music therapy capacities; and 4. At least two letters of reference must be submitted supporting this nomination. Recipients of this award will receive one-year student membership with the CAMT. Agency/Individual Citation The CAMT recognizes agencies and non-music therapists who have provided employment and promoted the advancement of Music Therapy in Canada. This award may be made annually to not more than one agency or individual. Qualifications: Any institution, organization or non-music therapist whose efforts have promoted Music Therapy. These efforts may include: 1. Has demonstrated a commitment to the CAMT through public relations and fundraising ventures; or providing employment, educational or training facilities; or promoting research in the field. 2. A letter of reference or another significant piece of supporting documentation must be submitted with this nomination. The recipient of this award will receive a two-year honorary CAMT membership.
