You must reserve your placement with a non-refundable deposit of $250 toward the tuition for the course.
This deposit covers salaries and other expenses related to contacting school districts to obtain your placement.
This deposit will be applied to your student account in partial payment of the tuition for the course during the
Course Time Period indicated above.
Failure to attend the class will result in forfeiture of the deposit and the necessity of paying another deposit
to take the course in the future.
In order to be registered for EFC 330, you must do the following.
1. Pay in person at the Cashier’s Office no later than the due date listed above.
2. Bring this notice to the Cashier’s Office. Be sure to tell the Cashier:

3. If you do not pay by the due date, or your payment is made incorrectly, you will not be registered for the
class, and will need to take it at a later time.
4. The Office of Field Experience will acknowledge receipt of payment within one week from the due date.
By submitting this form and paying the deposit, you agree to the following.
1. The Office of Field Experiences is authorized to register you in EFC 330 for the time period indicated
above.
2. Should you not attend EFC 330 as scheduled you will have caused additional work for the university
and school district, and potentially damaged relationships that are critical for the success of other
students.
3. The deposit is non-refundable, and will only be applied to tuition for the time period indicated above. It
is not transferable to another period.
4. You are responsible for checking your CWU email regularly for updates regarding this class.
NOTE:
1. The tuition shown in Safari will be the entire tuition for the 2 credit course prior to applying the nonrefundable deposit.
2. The non-refundable deposit when applied toward the course tuition is eligible for financial aid.
