FIRST RECONCILIATION AND FIRST EUCHARIST REGISTRATION
PLEASE READ:
A baptismal record with notations must be submitted to the office by October 31,
2015 except for those who have been baptized at Prince of Peace Catholic Church. Your child must be
enrolled and attending Christian Formation by December 1, 2015. If not, the sacrament would be deferred
to the 2016-2017 school year. There is a $25 Sacramental Preparation fee which must be paid when this
form is turned in.

All Parents are required to bring nutritious refreshments (finger foods and drinks) to each Module. Keep
in mind our children will be eating lunch together. We do try to maintain as complete a peanut-free
environment as possible, so please, NO PEANUTS!
Both the parent (grandparents and godparents may substitute) and the child must attend each module.
Unfortunately, we are not able to provide child care. Since this is a special time for the child preparing to
celebrate the sacrament, we ask parents to please make child care arrangements for siblings.
