Practice Profile
The Reddish Family Practices are located in the northern suburbs of Stockport, a town which comprises a polarised population with pockets of significant deprivation and considerable affluence an affluent town to the South East of the Greater Manchester conurbation bordering the Derbyshire
Peak District to the East and the Cheshire Plain to the South.
The Family Surgery is situated in North Reddish, 3 miles North of Stockport town centre, and South
Reddish is situated in South Reddish, 1 mile of Stockport Town Centre a Stockport Ward bordering both Tameside and Manchester. The patient population is drawn from an area, which can be described as industrial. Surrounding housing stock consists primarily of traditional turn of the century properties, primarily terraced and semi-detached dwellings, which leads to a practice list comprising young families and an older retired population. The practice population exhibits many of the health problems associated with an inner city practice.
Our practice serves approximately 12,000 patients who will from April 2015 be cared for by 6 GP partners, (three male, three female). The current vacancies have arisen following the retirement of our long serving senior partner. Two current salaried GP’s will become a partners from April 2015 and we are looking to recruit to fill these positions.
North - The practice moved to purpose built premises in 1990, having previously occupied traditional converted domestic dwelling premises. In 1996 the practice was extended to provide additional administrative space upstairs along with an additional consulting room at ground level. We have currently undergone some minor renovations which has enabled us to offer ground floor accommodation including 5 consulting rooms each with their own treatment area, a suite of rooms for the practice nurses, Health Care Assistant and a new Trainers room. Upstairs accommodation consists of a kitchen/common room, a large administrative room and the practice manager’s office.
South – We started to work closer with South Reddish Medical Centre following the retirement of
Drs Cole & Benjamin to enable us to extend the range of services we could offer to patients. South is purpose built premises which comprises of 6 consultation rooms at ground level. Upstairs accommodation consists of a Kitchen and a large administrative room.
The practice offers a full range of GMS services with the exception of Minor Surgery (which we are hoping to start offering in April 2015 after CQC approval) . The practice provides maternity services
Child Health Surveillance services. The practice achieves top target levels for Cytology screening
(>80%) and Childhood Immunisations (>90%).

The practice has a history of long serving staff members and most of our current staff members have worked at the practice for more than 10 years.
The practice has a well-established IT infrastructure and is a paperlite Microtest user.
NHS Services
We are a PMS practice providing Primary Care Services to our primarily Stockport registered population. Stockport has been early implementers of Commissioning and our practice is keen to explore the opportunities offered by this change in structure of the NHS. The Practice has opted out of out-of-hours (OOH) provision and these services are presently contracted by the PCT in Stockport through Mastercall Healthcare Ltd a company established from the Stockport OOH Drs Co-operative.
The practice provides extended hours across in the early mornings and on one Saturday morning per month. All GPs take part in this on a rota basis. In addition, the Practice is fully involved in providing
Enhanced Services, Public Health Vaccinations, spirometry, Coils & Implants and ECGs etc.
Additional Activities
The Practice aims to be involved in helping to develop tomorrow’s doctors. Dr Nurcombe completed his training which has enabled us to provide placements for FY1 doctors and we hope to increase our involvement in this aspect of work across the next 3 years.
Personnel
The practice is managed by a full time Practice Manager who attends to the day to day running of the practice including QoF and IT development and who is supported on a sessional basis by a part time Business Manager who has responsibility for the practice finances and the strategic direction of the practice.
The practice engages the services of a Nurse Practitioner, a Practice Nurses, and a Health Care
Assistant.
To support our clinicians the practices employs 13 further staff in various support roles including reception, administrative, secretarial, and audit.

The Practice is supported clinically by PCT / Hospital Trust employed Health Visitors, Community
Nurses and midwives.
Patient Involvement
The Practice has established a Patient Reference Group and started to undertake patient surveys.
The practice has also established its practice website which enables patients to order repeat prescriptions online and on-line booking of appointments.
Other Achievements
The practice has performed well in the Quality and Outcomes Framework (QOF) as well as National
Patient Satisfaction surveys.
IT
The practice utilises Microtest Medical Software along with Microsoft Office applications.
2. Job Description
The post-holder will manage a patient based caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. As a senior member of the team they will be expected to set an example to others, assist in education and training and to play a full role in target achievement and service development.
Clinical Responsibilities:
• The GP will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home and in nursing homes, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence as required.
• Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
• Screen patients for disease risk factors and early signs of illness
• Cover all the clinical management of the patients in surgery as appropriate including initiating investigations, reviewing results, making referrals to secondary care or to other providers as appropriate

• Develop care and treatment plans in consultation with patients and in line with current
Practice disease management protocols as well wider guidance such as that from NICE
• Ensure appropriate follow up of patients
• Record clear and contemporaneous computerised consultation notes to agreed standards
• Collect data for audit purposes
• Compile and issue computer-generated acute and repeat prescriptions avoiding handwritten prescriptions.
• Prescribe generically whenever appropriate and in accordance with the Practice prescribing formulary, NICE guidance and Good Prescribing Practice (as defined by British
National Formulary guidance)
• Work with and support other members of the primary health care team in delivering high quality primary care
• Promote the application of standards and codes of Practice set by the Royal College of
General Practice, General Medical Council, Department of Health, and local NHS trust
• Play a lead role in the practice achieving agreed QOF and other key performance indicator targets.
• Take lead responsibility for targets and services allocated, e.g. a QOF area or an Enhanced
Service.
Other Responsibilities within the Organisation:
• Awareness of and compliance with all relevant Practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety
• Commitment and contribution to achievement of the highest possible quality standards such as the QOF and other targets
• A commitment to life-long learning and audit to ensure evidence-based best practice
• Contribute to evaluation/audit and clinical standard setting within the organisation

• Attendance at regular in-house governance and educational meetings and training as well as events at other agencies as appropriate

• As a GP you will have access to confidential information relating to patients and their carers,
Practice staff and other healthcare workers. They may also have access to information relating to the
Practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
• Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

The GP will assist in promoting and maintaining their own and others’ health, safety and security as defined in the Practice Health & Safety Policy, to include:
• Identify the risks involved in work activities and undertaking such activities in a way that manages those risks

4. Terms and Conditions
Annual salary based on number of sessions (4/8) worked per week and experience
NHS Pension
Personal Medical Defence fees paid through practice.
Holiday entitlement (including study leave): Six weeks pro-rata equivalent
Flexibility will be required on timing and venue of sessions.
To facilitate communication, clinical update and management tasks, you may be required to attend evening meetings and other events that fall outside normal practice hours.
