Patient Information – MyOnlineClinic Pilot Trial 2015 Thank you for your interest in trialing MyOnlineClinic the app that gives you access to your GP at the click of an appointment button. MyOnlineClinic is an app that has been developed by Telemedicine Australia Pty Ltd with support from the University of NSW and NSW Trade & Investment. MyOnlineClinic uses the latest video technology, Bluetooth devices and file-sharing without the need for any additional software. With MyOnlineClinic you can see your doctor from home or from wherever you are in Australia or even around the world. Watch the MyOnlineClinic Video Go to www.YouTube.com and search for MyOnlineClinic About the Pilot Trial The trial, also called a Pilot Trial, is designed to test the MyOnlineClinic app and to get your feedback. MyOnlineClinic Pilot Trial recruitment is being coordinated by the team at e-General Practice Solutions www.e-gps.com.au Phone: 1300 990 863* Taking Part in the My Online Clinic Pilot Trial Registration 1. Sign and scan the “Patient Participation form” (second form attached) and email back via replying to this email. Or Respond to this email by writing that you have read and agreed with the terms of patient participation form”. Please make sure your full name and address is in the reply email. 2. By signing the Agreement you agree to take part in the MyOnlineClinic Pilot Trial for a period of six months. 3. Once we receive the signed Participation Agreement, the MyOnlineClinic training team will send your log-in detail to the website. The Equipment 4. To take part in the MyOnlineClinic Pilot Trial, you can use your medical devices or you canpurchase equipment that is Bluetooth compatible eg; blood pressure cuff, thermometer, glucometer to take a blood glucose reading 5. You can share the Bluetooth equipment with others who are using MyOnlineClinic. We suggest getting a family group, sporting club or social club together to invest in the MyOnlineClinic equipment 6. Once your group has purchased the Bluetooth medical devices the MyOnlineClinic training team will show you how to use the equipment, how to upload your data and how to book a GP appointment. 8. After the training session, you will be given access to the MyOnlineClinic website and you can start uploading your data. 9. Also you have access to Live-chat support via the MyOnlineClinic website. (Orange button in the top left hand side) The Pilot Trial 10. Over a period of 6 months you will be asked to upload data about yourself (eg; temperature, blood pressure, blood glucose). 11. We ask that you book and take part in at least one online GP appointment via video link. 12. You will be given two free MyOnlineClinic consult vouchers, so your first two GP appointments as part of the Pilot Trial are free of charge. However, please note that once you have used up your vouchers, your GP may charge you for any additional GP appointments using MyOnlineClinic. 13. You will be asked to record your experience of MyOnlineClinic in the easy-to-use Patient Report 14. At the completion of the trial the MyOnlineClinic team will send you a report of the Pilot Trial findings Frequently Asked Questions Why is the My Online Clinic Pilot Trial necessary? All online applications need to be tested before they are released to the public. The University of NSW and Telemedicine Australia Pty Ltd have designed this Pilot Trial to get feedback from both doctors and patients as to any issues which users might encounter when using the system. That way the MyOnlineClinic system will be safe and easy to use for patients in the future. The Pilot Trial isn’t about your medical information or treatment - it's about the MyOnlineClinic system you are testing. What is my role in the My Online Clinic Pilot Trial? Your role is to test the app by uploading your data eg; blood pressure, temperature, blood glucose via Bluetooth compatible medical equipment. You must also book and take part in at least one online GP appointment via video link. You record your experiences in the easy to use Patient Questionnaire. Who will see my personal information? You are in charge of your personal data. The information you upload into your secure patient file is only visible to the GP with whom you have your consult. No other party involved in the research will have access to your personal data. Do I need to buy the Bluetooth equipment? Not neccessarily. The MyOnlineClinic Pilot Trial has a limited number of kits for participants to use. The kits will need to be shared for the duration of the trial. To ensure you have access to your own equipment, we recommend that you get a group together; such as a family, social club or sporting club to purchase equipment that is Bluetooth compatible eg; blood pressure cuff, thermometer, glucometer. That way you can share the equipment and look after your health together into the future. The My Online Clinic support team will provide a full training session on how to use the Bluetooth equipment, how to upload the data and how to book and be part of online GP appointments. What if I decide I don't want to take part in the Pilot Trial anymore? Your participation in the MyOnlineClinic Pilot Trial is voluntary. You can withdraw at any time.
