Notes for Completion of COSHH Form
The COSHH will be done by you as project supervisor. You should use the COSHH
assessment form below which is available in an electronic format.
All the relevant information is available from the MSDS provided by chemical
suppliers. One COSHH sheet should be used per chemical. ‘High risk’ procedures
should be reviewed every two years, with others every five years.
For some staff members, who are not involved in any potential chemical or biohazards, one ‘nil return’ on your COSHH will cover all your project students (although
the student still has to complete the Risk Assessment).
Once completed, the assessment should be discussed with each student to make
sure they understand it and the precautions required. A copy should then be
retained by the student (for inclusion in their final report), the supervisor and in the
Department (submit to Head of Department).
A separate form should be completed for each Hazardous Substance used.
