1. Need for Program. Describe who the target population is and whether their numbers are
increasing or decreasing. Indicate the source of this information. Also, indicate how the
program/service is different and how it is similar to others and what would happen if the
program/service ceased to exist. List the priorities established by ASSET Funders, which the
programming addresses.
____Target population includes those in need of mental health services in Story County. Last
year 6000 people in central Iowa used services provided by the clinic, 1056 people from Story
County. Next year’s projection is that people who will use service will increase from 1056 to
1150. –From 2016/17 Budget Request and personal interview of Kathy Dinges, agency director
on 10/2/15.

2. Program Strengths. Pick two to four factors that contribute most importantly to the
program/service outcomes. Do not list everything that is satisfactory. We will assume that
things not mentioned are okay. For each strength describe some supporting evidence.
1. Provides Psychiatric Evaluation for patients; prescribes medication and follows up
with patient. Served 251 participants last year.
2. Med Management, confirming proper dosage and use of medications for those with a
psychiatric disorder. Served 1938 participants last year.
3. Mental Health Evaluation. Acquiring information to create a mental health treatment
plan. Served 527 participants last year.
3. Program Weaknesses. Select factors that detract most from the program/service achieving
its outcomes. Present details as described above. Recommendations for reducing these
weaknesses should follow the discussion of each weakness.
1. Difficulty in hiring and retaining medication providers, those who prescribe medications.
Trend is “tele-med” perhaps offering less service without decreasing costs.
2. Difficulty in hiring and retaining qualified therapists that are certified for each insurance
provider.
3. Difficulty in aligning treatment methods and desired outcomes among those who need
services, their families, and the courts
4. Financial Outlook. Current funding concerns should be described. In addition, the
assessment of the program's plan to cope with shrinking resources should be given.
1. Unclear Medicare reimbursement outlook.
2. Unclear local funding outlook.
3. Able to adjust programs and prioritized based on actual funding received.
5. Internal Management Practices. Summarize your assessment of the management of the
agency, and the role of the Board. If there are deficiencies, details should be provided.
Conclude with recommendations when appropriate.
No concerns observed. Director is on third year at Eyerly Ball, but has significant administrative
and therapeutic experience prior to Eyerly Ball. The agency has practice of actively seeking
feedback from clients and is getting positive feedback.
6. Agency Suggestions/Comments. Make note of any suggestions, comments or questions
agencies may have with regard to the ASSET process, budget forms and or anything else that
relates to the process.
GENERAL ASSESSMENT:
Eyerly Ball seems to be a strong resource for those who have mental health needs in our
community. They have significant history, able staff, facilities, and reputation to contribute to
meeting these needs in our community.
