Please read carefully the “Instructions for Local Federations” before completing the application.

Please complete Attachment A – Federation membership listing
Please include a hard copy with your application and also e-mail it to cfc@bostonabcd.org.
Please note:
Federations must submit a Local Independent Organization application for new member
agencies.
You do not need to submit an application for federation members who participated in the 2015
CFC.
Completed Applications and all required attachments must be mailed to the address below.
