Instructions:
Section A: Upon receiving a coaching request, contact the supervisor to begin the process of clarifying skills needing to be further developed and document the
skill area and competencies in this section. In some cases, the RTC has already provided this information.
Section B: After gaining clarity regarding the skill(s) needing further developed, arrange a meeting with the supervisor and learner to collaboratively identify
desired practice behaviors and ask the supervisor and learner to rank current behavior.
Section C: Develop a coaching plan by completing Section C. The plan becomes the blueprint for your coaching activities. As with case plans, they may need to be
revised or renegotiated.
Section B and D: Upon completion of the coaching plan. Have the supervisor, learner and you rank desired behaviors in Section B to indicate progress and
provide a developmental plan for skills still needing further development.

Responsibilities of the Coach






Demonstrate respect for the learner’s perceptions and learning style
Help the learner clarify skills needing to be developed
Provide objective assessment and observations
Maintain documentation of coaching activities
Maintain confidentiality
Provide notification if unable to keep appointments and reschedule at learner’s earliest convenience






Collaborate with coach and supervisor in the development of the coaching plan
Make all appointments on time, or attempt to reschedule ASAP if an unavoidable conflict arises
Complete all assignments and activities as outlined in the plan
Utilize the tools, concepts, models and principles provided by the coach to enhance identified skills
Assume full responsibility for personal decisions and actions
Responsibilities of Learner’s Supervisor




Collaborate with coach and learner in the development of the coaching plan
